Protocol for Osteoporosis treatment with Evenity and Reclast

My endocrinologist strongly urged me, after years of Prolia treatment, to do this next step: a full year of Evenity followed by two infusions of Reclast, one year apart. He says the Reclast is necessary to seal the good effects of Evenity.

I had good outcomes with the Evenity (according to him, my bones are now in the condition of a 30-year old... I'm not sure I believe that, but...)
The Evenity treatment ended last December and then had the first Reclast infusion in January of this year. I don't think I had any side effects, though it's hard to know, but nothing obvious. Still, I'm wondering whether it's really necessary to have the second infusion this coming January.
The doctor insists that this is the protocol, but I still wonder.
Has anyone else had experience with this?

While your endocrinologist is correct about protocol the gentleman who ran the clinical trials says he rarely ever give the second year of Reclast. There are serum bone markers that would clarify the need or lack of for the January infusion CTX and P!NP. CTX alone might give you the answer. Bone markers aren't protocol either.

Protocol according to my endo is to check CTX. Mine is still low 18 mos after first Reclast infusion so no need for second infusion (yet). In meantime, walking, swimming, and doing Onero twice a week. Will check CTX again when I have my next Dexa in Jan 2027. Best wishes!

Two reclast infusions 1 year apart is the protocol. Should it be for all of us? Probably not but who knows! Just had my second infusion 2 days ago (so far so good, fingers crossed!). Had a CTX before the second infusion; showed the "medicine is working" per doc. No DEXA, protocol for that is every two years with the practice I am with, so mine will be in summer '27. CTX was 52 after a year of Fosamax (no baseline testing before starting osteoporosis meds), 78 after 1 year of Reclast. Have no idea what exactly that means, but I think it is low?

I'm a few months from finishing my year of Evenity. My rheumatologist had told me I will start Reclast after this. She told me a once-a-year infusion for 5 years.
