I was in a similar situation, living in the Yucatan peninsula of Mexico. I had the same stage as you and was Gleason 8. I went to Mexico City and consulted a very reputable surgeon at a top notch hospital. He’s a surgeon so he recommended surgery. I had a PSMA scan that showed seminal vesticle invasion, but the surgeon convinced me he had a good chance of getting it all. Well he didn’t. So I ended up traveling to Houston and received IMRT radiation at the Dan Duncan cancer center. So far so good. The advice i would give is seek out a facility with latest generation radiation equipment and a great Radiation Oncologist. I believe that a well planned treatment delivered by newest generation equipment can give you the best results with the least collateral damage/side effects. I have seen many studies where Australian doctors and facilities were involved. I’m confident there are some good options there for your treatment.
- surgery will remove all of the cancer inside the prostate, but will miss any that's spread outside locally and hasn't been detected yet (that seems to have been what happened in your case, if I'm understanding correctly)
- less-focused radiation therapy can attack cancer near the prostate as well as the cancer in it, but comes with more side effects (because the radiation does collateral damage as it spreads)
- tightly-focused radiation therapy will have the fewest side-effects, but might miss some not-yet-detected cancer other places inside the prostate, as well as any local spread adjacent to the prostate
So, as I've mentioned before, there's no slam dunk here. They all have comparable overall-survival rates, and none alone is guaranteed to get everything. Some people follow a prostatectomy with post-op "salvage radiation" to have a better chance of catching any undetected local spread, but then you get both the surgery *and* radiation side-effects.
It's a bit like a version of Let's Make a Deal where there's a goat behind every door. 😕

What I'm finding is different people with variety of situations.
It's a bit like that woman goes to the shop to find a husband and on each level a different option and she can't go back to a previous level.
