PC in family history, monitoring PSAs since 2005. Results jumping around 2014-2024 based on medications, injuries, etc... March 2025 off ALL meds prior to testing. Requested Prostate Health Index rather than standard PSA - new testing recommend by highly rated surgeon re: gall bladder, gi issues. March 2025 requested PHI - primary care never heard of it but ordered anyway came back 5.82. Requested & performed Prostate MRI. Proceed with biopsy - Perineal Biopsy (less risk for infection) access in sphincter area (between 2 regions) Perineal biopsy with "overlay" of MRI images created more accurate testing locations. 19 biopsies; 9 positive. Gleason Score 3+4 & 3+3. On to PSMA pet scan. Onward to URology. Recommended RP. Onward to Proton Therapy RO - recommended ADT & Proton Therapy. This appt was 2 weeks AFTER biopsy - standard PSA came back 8+. 08/06/25 another standard PSA test came back @ 4.94. Received ADT injection with Oral med daily for 60 days. Proton Therapy to begin in 1 month. History and now the question: Decipher test based on PSA 8.64 (while still healing from biopsy). 2 months later back down to 4.94. Wondering if PSA results has large impact on Decipher results as "high Risk." Still dont really comprehend the Decipher test/results. Not a fan of ADT but doing with with monthly injection for 6 months -read the lower monthly dose creates less of the symptom "flares" and if it too bothersome to continue; 1 month injection vs 3 or 6 month injection allows better management control

There is no real benefits to the one month shot over a three month shot. It’s a Real pain to have to go in every month. If they’re given you Firmagon, it’s not only a pain going. It’s a pain getting the shot. I got six month Lupron shots for seven years, I actually tried a three month shot to see if it made any difference, It didn’t. Had no issues/pain from the shot itself, And the side effects of ADT have been identical between Lupron and Orgovyx, Which I switched to two years ago.
The flare only occurs the first month you get the shot, And if they did it right, they gave you Casodex Two weeks before the shot or started off with Firmagon or Orgovyx, which don’t have the flare problem.
