Pathology report is in 😮‍💨

I was diagnosed with Gleason 4+4=8 in late December 2024. Had RARP on 06/16/25. PSMA was negative and Decipher 0.2. My last PSA was 5.5.

Pathology:
Negative surgical margins
Gleason 3+4=7
Stage T2c
No SVI
No LNI
Cancer involvement 11-20%
No adverse features such as Cribriform, IDC, or pattern 5.

Hugely relieved. Surprised by the downgrade from 4+4 to 3+4 (After Gleason 8 diagnosis I went into a mental tailspin of anxiety and depression).

Only fly in the ointment is that the stiches did not heal and that I will have to wear the catheter for another 10 days. 🤦

Good news about downgrading.

As for pathology grade matching needle biopsy grade —> Though study results vary somewhat, studies between 2008 - 2023 indicate that initial grade and pathological grade match much of the time. This 2019 paper of a large study out of the UK indicated that initial biopsy and pathological grade matched 59% of the time; upgrades occurred 25% of the time; downgrades occurred 16% of the time: https://bmcurol.biomedcentral.com/articles/10.1186/s12894-019-0526-9

(I was diagnosed with 3+4 in 2020, so we scheduled proton radiation; a 2nd opinion upgraded the 3+4 to a 4+3, so we added 6-months of Eligard to the treatments that I had during April/May 2021.)

TOPF - Nice pathology report with a down grade in Gleason IS solid good news . Any other symptoms fro you surgery ? What is your age and what was the PSA going into your surgery ? Stitches healed now - should be ... God Bless Sir . James on Vancouver Island .
