You may want to get evaluated by a urologist and primary doctor to get to the bottom of the cause of your pain. You want to make sure there is no issue with your kidneys and ureter.

Depending on what’s found, you may also want to also see an orthopedic spine/hip specialist to see if you have any nerve compression that can affect your scrotum.

My understanding from AI is that the nerves that directly affect the scrotum include the ilioinguinal nerve, the genitofemoral nerve, the pudendal nerve, and the posterior cutaneous nerve of the thigh. These nerves provide sensation to different parts of the scrotal skin, while other organs sharing nerve pathways, such as the kidneys or ureter, can also refer pain to the testicles and scrotum.

You didn"t give much info about yourself like age, when it began, etc. Hope is right you need to be evaluated. Are your testes are painful on exam or do not feel normal (no bumps, knots, specific sore spots)? Or is just the sack?
