Pain Flares Post Pluvicto Injection
Been through the first 3 Pluvicto injections. The initial treatments have not given problems but between 3 and 5 days following treatment I've had severe "flare ups." Each time has been in a different body part but they are similar in that each has had excruciating pain controlled only by substantially increasing the Oxycodone that I'm taking. Have other Pluvicto patients had similar situations? I'm especially interested in hearing from patients who have moved to #4, 5 or 6--should I expect a similar problem with them?

I see that no one has answered you yet and thought I would weigh in. My husband had all six Pluvicto infusions in 2024. He never had severe pain flares after any infusion. I notice that you are taking oxycodone, and it sounds like you were already on it before you began Pluvicto, which suggests that you had preexisting pain of some kind. I think it would be sensible to ask the doctor treating your preexisting pain if Pluvicto could be causing the severe pain flares, and to also ask whoever has ordered your Pluvicto if he or she has any insights on what the connection might be. Good luck, and please keep us updated.
The Radiology Team tells me that flares are a relatively common side effect at least in the early injections. You are correct that I've had a variety of pains from the metastization and was using oxycodone prior to Pluvicto. After the first two flares, things calmed down so I hardly needed oxy. My last treatment was Friday and the flare was Tuesday/Wednesday. We'll see how it goes from here. Thanks for your thoughts
I am one of the lucky ones. I had my 4th Pluvicto infusion on June 26.
My PSA has dropped like a rock from 80 to 5.5 and I have not had any
serious side effects. No pain at all.
What does your doctor say about your pain?
I am glad that your radiology team has verified that pain flares are a relatively common side effect of the early injections. Ask them whether they think the flares will subside or decrease in severity over the last three infusions, and whether there is anything they can do to minimize them. Sending good thoughts to you; let us know how it goes.
