How long were you on Orgovyx, Xtandi, Talzenna? Fatigue unbearable

How long do you normally stay on these three(orgovyx, xtandi, talzenna). I've been on a couple of months and my fatigue is almost unbearable. Anyone have this experience?

As @northoftheborder says, what you are describing could be a result of advanced prostate cancer to the spine.

On the other hand, I have on and off bouts with lower back pain, But I’ve had it for over 25 years, Way before the cancer. I have a belt I can put on, and within a day it normally makes my back stop hurting and I’m back to normal for another six months or longer.

So the real question is, is your lower back pain a common problem that many men have or is it related to prostate cancer?. Do you feel anything Going down your legs when you’re having this lower back issue, it can cause sciatica.

You could talk to your doctor about doing an MRI, a PSMA pet scan probably won’t find anything since your PSA is so low. A CT scan might work as well if they’ve got one to compare it with. This is something you have to talk to your doctor about. It may involve an Orthopedic MD.

These days you have to be own advocate. I had a colonoscopy and I looked at the results on my pt portal which showed a mass that needed further attention. I told my gastros Dr's nurses to make a referral to my urologist and then PET and here I am.

Well here it goes. Just found the cancer I have is aggressive and prior to nubeqa and shots has spread from prostate to the entire pelvic area and northoftheborder was right its probably meststatic to my spine. Called PCP I'm to see the Urologist on the 12th im.assuning the will do a PSA and nothing much else they can do

It's seems like I need to see an oncologist. Well hopefully my PSA will be lower than last time went from 53 to 19 after my first injection.

It's not necessarily true any more that there's nothing you can do once you learn you have metastatic prostate cancer. There are lots of options in 2025:

1. I think you're already on ADT + Nubeqa (Darolutamide). As a combination, they may stop the cancer from spreading any further for a long time just on their own.

2. If there are just a few metastases (oligometastatic), they may offer radiation therapy for the individual metastases. That has shown huge
survival benefits in recent studies. Ask about "MDT" (metastasis-directed therapy).

3. If there are many metastases (polymetastatic), they may offer chemotherapy and/or Pluvicto to hit the cancer early and hard. Ask about "triplet therapy."

4. They may offer radiation to the prostate itself if you haven't already had that (or a prostatectomy). Ask about "primary tumour radiotherapy."

Taken together, these new approaches to treating metastatic prostate cancer with "curative" intent have been revolutionary for patients in our situation: some oncologists even believe metastatic prostate cancer is on the verge of becoming a long-term chronic disease that can be managed indefinitely.
