How long were you on Orgovyx, Xtandi, Talzenna? Fatigue unbearable

How long do you normally stay on these three(orgovyx, xtandi, talzenna). I've been on a couple of months and my fatigue is almost unbearable. Anyone have this experience?

Some people do get excessive fatigue from Xtandi. In those cases it is a real solution to switch from Xtandi (apalutamide) to Nubeqa (Darolutamide). Ask your doctor about this. Your doctor should know that there is much less fatigue with Nubeqa.

Nubeqa has Fewer side effects than Xtandi. It also doesn’t pass the blood brain barrier so you get less brain fog.

I just realized that you were on a PARP Inhibitor (Talzenna). They are only usable for people that have BRCA in either their hereditary or somatic tests.

Do you have BRCA2 or BRCA1? If not, then the PARP Inhibitor is doing nothing but costing money and your side effects due to it. It does have a lot of issues with your white blood cell counts and hemoglobin.

I have BRCA2 And plan on using a PARP Inhibitor when Darolutamide fails me.

If you do not have BRCA, you should get off of Talzenna. I would be very suspicious of any doctor that prescribed this for someone that doesn’t have BRCA, Since that is the only thing it works with. Some doctors have tried to use it for people that have ATM, but it does not work.

There have been clinical trials investigating using PARP inhibitors in combination with ADT+ARSIto treat mCRPC, regardless of germline mutations, but I don't think any has made it into mainstream care yet.

Now that’s a good one. I’ve seen previous attempts at combining PARP, but it didn’t help at all.

This study really seems to show there is a benefit in PFS (progression free survival). He does have BRCA So it is quite appropriate either way.

It’s interesting that Jansen would make Akeega PARP+abbie in a single pill about the same time as the study results. I was actually paid to review the early Akeega Documentation. There were a lot of errors and incomprehensible stuff, But one thing it made clear, There were some major side effects. My oncologist had warned me that the side effects were too much, but this documentation went into detail about the major ones and what to do when they occurred.

That study was done in 2022. We definitely have not seen a rush of doctors to treat with Abbie plus a PARP. Why has this study not been followed up by More acceptance of PARP. Side effects?

And with one UCSF study this came up. PARP inhibitors can lead to a few serious side effects, explained Dr. Agarwal. These include a drop in blood cell counts, nausea and vomiting, and fatigue. Talazoparib, he noted, tends to cause substantial drops in red and white blood cell counts. To get around this, they reduced the dose in many cases.

Talazoparib PARP inhibitors works well with Enzalutamide says one Pfizer study. Which does seem to test a lutamide along with a PARP with is positive results?.

Are you saying you are getting hot flashes that are causing night sweats as well as day hot flashes?

I do have solutions for this, but you have not clearly stated what you need help with.

Sweating at night aren't hot flashes. I seem to sweat during the day without hardly doing any thing. Thanks for responding

If you are on ADT And are getting frequent hot flashes they can be controlled a couple of different ways.

I have an embrlabs.com wave product (wave 2). I’ve used it for Over three years now. It’s like a refrigerator that looks like a watch and sits on the inside of your wrist. You could set up one of the buttons for their night mode . Hit the button twice and It Produces cold waves at measured times during the night and prevents hot flashes and night sweats. They used to bug me, before I got this device . When you start to feel a hot flash coming on, you hit another one of the buttons twice and it sends cold chills through your arms and it reduces the intensity, shortens the life and can stop the hot flashes if hit quickly enough. If the hot flashes are real intense, it may not work, but it will work with the night sweats.
