Obtaining documents from doctors: What reports are needed?

As we all know getting documents from doctors can be tough. A family member has recently been diagnosed with breast cancer and is trying to get all medical records and reports together from the doctors to send it into Mayo.

(1) Based off of your experience, what documents are essential in the beginning to get things going? Right now we are trying to get the biopsy report, addendum, the order for imaging, and any/all health records including the clinical notes.

(2) Also, any advice on how to expedite or simply retaining the documents would be gladly accepted.

Hello, @austina . Is the appointment at Mayo already scheduled? When I was referred to Mayo for endometrial cancer by my doctor, the following was sent: The pathology report and the actual slides (biopsy) from the pathology lab. The Mayo pathologist reviewed the slides prior to my appointment in oncology. Your family member could call the Mayo department and ask what documents/slides/images they would like to have in her record. Does this help?
