Nine Months Into Recovery

I am nine months into recovery from HDR Bracytherapy and External Beam radiation. My first dose of HDR was September 10, 2025. My recent PSA is <.01 or undetectable. That is the good news. My first HDR procedure did not go well. Half of the radiation catheters came loose and they were reinserted without pain medication. The discharge process was also painful. My catheters were removed without the proper doseage of pain medicine. After my discharge from the hospital I thought there was no way I could tolerate another procedure two weeks later.
I spent the next 10 days in extreme discomfort. Spasms every two-three hours and then painful discharge into my pad. I called the oncology center, and was told my sypmtoms were normal and I was advised to take more Ibeprofen for pain. I also took a UTI test which was negative. Since I was urinating, I was never advised to go to an emergency room. On the tenth day my son found me incoherent on my couch and he took me to an emergency room where I was admitted with an overflowing bladder and acute kidney failure. My prostate had swelled, shutting down my urethra. I spent four days in the hospital.
I had to learn the art of self-cathertization, not much fun, but I got used to it. At its worst, I was using a catheter ten times a day. I was put on Orgovox to help reduce the size of my prostate. I finished my radition with 4 weeks of External Beam.
I stopped the Orgovox a month ago after taking it for 5 months. The side effects were bad. Lethargy, joint pain, hot flashes, weak physically, etc. The medicine was prescribed to help shrink my prostate, not to treat the cancer. After regaining my ability to urinate, Orgovox was discontinued.
The past three weeks my urinating has improved. It is still painful and my bladder alerts me with a stinging pain when it is time. If I can't get to a bathroom, I will urinate into my pad.
My recent oncology appointment was good. I am now in the normal stage of recovery symptoms, it took me a long time to get here. I will now start meeting with my urologist to discuss my situation, and review what options I have if my urinary tract and bladder function do not self repair.
I was told by the cancer center, and medical staff that they had never experienced my situation. I have gone down the rabbit hole of researching my reaction, and discovered that my outcome occurs in less than 1% of patients.
The pain and suffering, hospitalization with a life threatening bladder and kidney failure, and incontinence have been tough to handle both physically and mentally.
While I am relieved by my recent improvement, I still have a long way to go, and I am anxious about whether or not, I will require some kind of salvage surgery to address my incontinence.
Even though I was aware of all the side-effects of radiation, I never thought I would wind up in this situation. I have traded my prostate cancer for a urinary tract issue.

I can’t say that I’ve experienced anything close to what you’ve experienced.

When I was deciding on which treatment to use (back in 2019), I had 3 high school friends who had brachytherapy; all 3 of them had serious urinary issues (and they told me that the guys they knew who had brachytherapy had similar issues). So, I ruled out brachytherapy from the start. (Ultimately, I had 28 proton radiation treatments; no adverse side-effects to mention.)

It looks everything that could go wrong did go wrong with yours. Good luck going forward.
