New Lump After Mastectomy

Received very upsetting news that a new lump on my mastectomy site is indeed cancer. My mastectomy was November 24 (3.8 cm, grade 3, one positive lymph node, invasive ductal carcinoma). The new lump is cancer, possibly in the chest muscle. I’m wondering if anyone who has had a mastectomy before and local recurrence in chest/chest wall could comment on their experience and what they did for treatment. I’m panicking and my MO/ surgeon can’t see me until tomorrow. I would like to have an idea of possible options for treatment

I had a saline implant after mastectomy 22yr ago. Was told never need a mammogram because all breast tissue removed. My original surgeon told me that only 2% of patients have this happen. She believes this will be a new primary tumor. Chat GPT said the pathologist has to compare biopsy results to figure out if recurrent cancer or new primary. She is taking my case to Tumor Board at hospital where all cancer specialists convene and discuss case. Breast MRI done today. Will get lymph node mapping at lymphedema clinic beforenext steps.

Also they recommended Tamoxifen as I was premenopausal 22 yr ago. I declined the med. my 3 sentinel nodes were negative at the time. This time my ER + weighs in at 91%. Already know that aromatase inhibitors are a hard pass for me. I have 3 herniated discs and arthritic pain due to disc disease. Also I am 70 years old. I am willing to listen to radiation oncology when it’s time but not there yet.

I also had a reoccurrence 15 yrs after a mastectomy. It was on the scar line and was visible. My implant actually saved my life. The cancer wasn't able to permeate the chest wall. After having the tumor removed, chemo and radiation, it is now 22 yrs.
