My PSA climbed 02, .04, .07 .15 .21 had a RP 2 yrs ago PSMA negative ?

I am Lou, After 2 years after prostectomy PSA was undetectable
.02, .04, .07, ..15 to .21 oncology asked to do another PMSA
Results were good No Uptake No Cancer
Question, I go to RO tomorrow for consultation what happens if he wants to radiate prostate bed Is that common? Thanks for your reply!

@heavyphil I have been trying to “man it up” because normally I can work through things but it seems difficult. I think I may have to get something, thanks for the comment. I was afraid it might be a year. At first they told me just radiation. When the decipher came back they added adt.

@pamperme Under normal circumstances, manning up is usually all it takes…but when your entire way of thinking is hijacked by an outside influence (ADT) you have to fight fire with fire.

Suggest you wait until PSA is above .50 at very least before taking another psma scan.

@heavyphil Thanks for your comments. Originally my Gleason score was 3+ 4.
I’m the patient who mention PSA climbed slowly and realize the numbers are low.
.02 .04 .07 .15 .21 then then back a fraction to .18
Trying to understand the numbers and what is Happening.. thus went to surgeon who performed RP robotically 2 years ago
He saw my #’s creep up
and asked me to visit his modality clinic.
Instead went to my RO for consultation and compared 2 pathology reports almost 2 yrs apart.
2nd report from PMSA was thoroughly discused
and like 2 yrs ago the RO was in favor of active surveillance and wait and see next PSA in 90 days for another follow up

@charlotte1216 I have a similar experience. RP in 2020. About 2.5 years later, PSA went from .1 to .28, .38, .42, and .78. The last 3 PSAs within about 3 months. My Gleason was 4+3, and at the time of RP, the cancer had spread to the bladder neck. When my PSA began to rise, they did a PSMA PET scan at .42 and found it had spread to the lymph nodes. I was told I was very lucky that it showed on the PSMA PET scan at PSA .42. Here is one study from URO Today - albeit from 2022.

Key Findings for PSMA PET Detection Rates:
PSA < 0.5 ng/ml: Studies show a detection rate of roughly 38%–42%.
PSA 0.5 – 0.99 ng/ml: Detection rates increase to 57%.
PSA 1.0 – 1.99 ng/ml: Detection rates jump to 84%.
PSA >2.0 ng/ml: Detection rates are very high, up to 86%–97%.

My point is that I would imagine they would repeat the PSMA PET scan if the PSA levels increase. My case now. After ADT, my PSA levels have begun to rise again, from .04 to .07 over the last 3 months. However, they won't repeat a PSMA PET scan until it reaches a much higher level. Wishing you the best.
