Multiple myeloma with T (11.14) translokation

My wife is 76 years old and was diagnosed with multiple myeloma on 04/25/2024. She is being treated with Isatuximab + Bortezomib + Dexamethasone. She achieved a VGPR after 6 months. However, since then, the treatment has stopped working and the doctor has discontinued it since the beginning of April 2025. A new chromosomal analysis revealed hyperdiploidy and a translocation T(11;14). The analysis also revealed some circulating plasma cells. The doctor has not ordered any new treatment and no CRAB symptoms (Hg is 11.5) are currently manifesting. The doctor believes that a Venetoclax-based treatment can only be considered in the third or fourth line ... Is anyone facing the same situation and should I seek a second opinion?

My husband has MM, just finished 6 months of treatment and is currently on a maintenance schedule. He sees his oncologist monthly, receives a monthly injection and takes revlivid daily for 3 out of 4 weeks monthly. He has monthly blood tests done to monitor his continued response which has been excellent. I would get a 2nd opinion. May husband is 79.

I believe he has the same translocation or wife does and we were told that could be a source of treatment later on.
