Anybody know where I can obtain either at no cost or resasonable cost?

We usually have blank copies at the hospital that people can fill out. Then it has to get notarized. That way, its pretty much free. I would think you would be able to find blanks online?

Can we access our Advanced Directive through our Mayo Portal? I took mine originally to the business office to be scanned, but I can’t find it in the portal.

That is an excellent question. I went out on portal see if I could find mine, no luck. I sent a msg to Mayo Online Support to see if they can answer that question.

Got the answer quickly, but finding was not as quick, but I did find mine. Problem is the list is by year, so if do not know year have to look at each year till find. I started at 2008, worked backwards and finally found in 2019.

If do not find with their first suggestion, then have to request copy.

I guess if in doubt, have Mayo scan it in again.

Here is Mayo Online Support response:
Legal documents (Advance Directive, Living Will, Power of Attorney): Select “Health Record” from the options at the top of your home page, and then choose “Clinical notes & documents.” Select “Documents” from the options at the left. If your legal document is on file with Mayo Clinic, you will need to select the year Mayo Clinic received your document in the drop down provided. If you are unable to access your document, please use one of the methods below to request that information.

To request medical records using Patient Online Services:

1. Log in to your Patient Online Services account.
2. Select “Health Record” from the options at the top of the home page on web, or from the bottom of the screen in the Mayo Clinic App.
3. Choose the “Manage health records” card and then select "Sharing Hub."
4. Select the “Request formal copy of health record” from the options at the bottom of the page.
5. Follow the instructions in the form to identify the records you wish to request.
6. Select "Submit Request" to complete your request.

@chrisbickes You can go to 5 wishes https://www.fivewishes.org/ and find the free/reasonable cost documents that you are looking for.

Good for you for thinking about this now, and working to get the paperwork completed! It will give you peace of mind.

I have an Advance Directive through my healthcare insurer (free). I also recommend the Medical Orders for Life-Sustaining Treatment (MOLST) if your state has one (free).

A medical Power of Attorney might require a lawyer. I have both a medical P. of A. and a financial P. of A. assigned to different people.
