if it is effective to address pain in feet from peripheral neuropathy. Should I expect to see a drop in levels of B-12 in serum? Should I begin supplementation now of B-12? What is consensus on this?

During my research of what to take for my polyneuropathy I learned from Dr Emily Splichal’s book Barefoot Strong that it has to be R-ALA instead of regular ALA: “Touted as one of the most powerful anti-oxidants, alpha lipoic acid has been shown to improve microcirculation to peripheral nerves while decreasing oxidative stress. A key point about alpha lipoic acid is that it must be taken in the R-lipoic acid form as the “R” form is the one that is biologically active (vs. “S” form).”

Initially I took it mornings and evening, but to be supportive of my silent reflux I now take the whole dosage in the morning.

I remebered reading that a racemic mix might be better , but I think I originally read it on a 'dot.com' site , so I gave it less credence since they were trying to sell product. But a cursory look provided this input:
" It has been hypothesized that R-ALA would be the most appropriate form of commercial supplement; however, it is prone to polymerization and is absorbed to an even lower extent than racemic-ALA. As a matter of fact, it has been demonstrated that the presence of the S-enantiomer in the racemic mixture prevents the polymerization of R-ALA to some extent, increasing its bioavailability [6]. For this reason, the best nutraceuticals or food supplements available on the market are represented by highly pure forms of food-grade racemic ALA in order to keep the polymer content at a low level ensuring stability over time and optimal absorption. The same study mentioned that liquid forms of ALA are probably more bio-available.

The doctor has me come in every 3 years for a scope and yes, he does a biopsy at the same time. Omperazole 1 per day 20 mg and sucralfate is 1 GM I take 3 times a day about 45 minutes to 1 hour before meals.

What’s the dosage of the ppi you take and how often and the dose and how often u take the sucralfate? How often do you have esophagus scopes with biopsies? . It’s super you are doing well. Thank you.

Sorry....my very last comment on this topic, I promise.
It seems that the positive studies all were done with IV drip administration of racemic ALA, less inf about oral use but it seems to get degredated pretty rapidly in the GI tract.

Encouraging that you have taken these two medicines so long and your Barrett’s has not progressed. I like that! Read that B vitamins and folic acid help the Barrett’s….so I am just starting that plus vitamin D.
