Hearing changes 9 months after partial stapendecotmy

I am desperately seeking answers to fix these hearing changes I have been experiencing post stapenectomy.
Oct 2023- partial stapedectomy on left ear. Unfortunately, scar tissue pushed the piston out of place.
March 2024 - revision of partial stapenectomy with long piston placed. I had amazing results afterwards. My bone air gap closed by 20 points at some sound levels! It took about 6 months to get used to all the new sounds and I even stopped wearing my right sided hearing aid.
Jan 2025 - I started to notice alot of noise distortion-specifically in the car, with water running, and certain vocal pitches. I also noticed fullness in my head and occasionally hear clicks and pops (without moving my jaw)
What I have tried so far -
-3 weeks of Flonase, nasal rinses, antihistamines, and decongestants.
- a paper patch to assist in a overly patent estascian tube
-a nasal scope - negative for obstruction to estascian tube
- high dose steroids
- intra oral massage
- PT of the jaw/facial muscles
None of the above has helped with any improvement in sound quality. My MD keeps talking about cbt and learning to ignore it as if it's tinnitus. It's is not. It's is only present with certain input and goes away when the stimulus goes away.
I can also give myself an underbite and I can hear PERFECTLY for a brief moment. I can also force pop my ears or create a brief vacuum by cupping my hand over the ear.
My latest efforts are focusing on vestibular migraines and I have little hope. I am e been taking magnesium and B2 and will also be starting on low dose topimax.
I am usually a pretty resilient person, but I have been dealing with this for over 8 weeks and I feel like I am sliding into a depression. I can't enjoy music, the theater, car rides in my convertible, and even my husbands voice at the end of the day. All of my coping mechanisms are the cause of my stress.
If anyone can offer advice on how I can move forward and get back to the glorious hearing I was blessed with for those 9 months.
