Have to use catheter to empty bladder. Reocurring uti's.

Looking for help for my 50 year old daughter. Bladder has to be emptied with catheter. Frequent uti's. She is living overseas and has a chauvinistic dr who blames her for not being careful about hygiene. I don't think he has done any investigation except cystoscope. He could not get in as I understand it. I read that bladder issues can occur during menopause because of hormone changes. She has begun menopause. She has several autoimmune disorders. She is scheduled to move back in August, but I hope that that is not going to be too long to be seen by a competent doctor. Anyone with this problem?

It is quite possible that perhaps she is not using sterile technique. Has he tried to instruct her on how to cath properly and disinfect the catheters that she is using?

She well understands the hygiene issue. She is highly educated and because of her other health issues is very careful about the process. He gave instructions but does not listen. His assumption is that it is her fault if she has a UTI . A condescending, chauvinistic attitude. She had to forcefully demand an antibiotic for the last UTI. I just hope she can get back to the states before it gets worse.

This is so disturbing and unnecessary.
D-MANNOSE in POWDERED form has been Life changing for me. I had frequent UTI's caused by e-coli bacteria for many years. I became anti biotic resistant to several antibiotics. It threw me into a deep depression. I have now been UTI free for a year and a half while on D-Mannose in powdered form. I found that capsule form did not work as it is not concentrated enough. Cranberry juice has done nothing for me. For an acute UTI, I take 1 teaspoon rounded (2000+ mg) every 2-3 hours in 1/2 cup of water, for 5 days. Try to wake up throughout the night to take your dose. Taking higher doses may cause diarrhea. Wait for 45 minutes to an hour. Then, drink plenty of water in order to flush out the bladder and get rid of E-Col. For a Preventive Dose, I take 1 teaspoon, two times a day for long term maintenance. I plan to do this indefinitely. Take D-Mannose as early as possible when you first notice symptoms, before the UTI fully develops. I buy PURE D-MANNOSE or NOW brands on from Amazon. I buy the bulk package as there is cost savings. D-Mannose is a simple sugar that is thought to block e-coli bacteria from sticking to the urinary tract and bladder walls. I only wish that I had discovered D-Mannose years ago. I discovered it while researching bladder infections on line where I found interesting studies by DR. OREOLUWA OGUNYEMI, MD.

You need 2000 mgs to treat an active UTI. Manufacturers of D-Mannose in capsule form have been getting away with this misinterpretation for too long. If you look at most bottles of D-Mannose in capsule form, it will say 1000 mgs or 2000 mgs on the bottle. However, look closely at the directions and note that one capsule is likely 500 mgs and that to get 1000 mgs you need to take 2 capsules. To get 2000 mgs, you need to take 4 capsules. Please, be careful and read the directions on the bottle.

Thanks, when I had colorectal surgery for cancer I went to the wrong surgeon. She left me with a colostomy and cut the nerve to the bladder which causes it to not empty so I have to self catheterize. No matter how much I wash with dial soap, use benzakonium wipes, a anti microbial perineal spray, wash my hands with dial soap, use a sterile catheter each time and try not to contaminate it (this is hard) I still get UTI’s frequently. I take cranberry pills with D-Mannose but not the dose you recommended. I saw urologist at Mayo yesterday and he started me on methenamine hipp. Has anyone else taken this? If a bigger dose of D-Mannose worked I would rather do that. Thanks, in advance, for any comment.

ONLY take D-Mannose in powdered form. It is far more concentrated than the pill format. 1 teaspoon of D-Mannose in powdered form is equivalent to 2000 gms of D-Mannose. I believe that most capsules have only 500 mg of D-Mannose. The labels on most bottles of D-Mannose capsules are misleading. The bottle may state 1000 mg, but to get the 1000 mgs, stated on the bottle, you need to take 2 capsules. To get the 2000 mgs, you need, you would need to take 4 capsules, three times a day. If your infection is active and as I mentioned earlier, you should take 2000 mg of powdered D-Mannose every 2-3 hours with 1/2 cup of water, for 5 days. Yes, you need to wake up throughout the night to take your dose. Taking higher doses may cause diarrhea. Wait for 45 minutes to an hour. Then, drink plenty of water in order to flush out the bladder and get rid of E-Col. Then reduce the frequency to a Preventive Dose of ½-1 teaspoon (I take 1 rounded tsp) one to two times a day for long term maintenance. I truly wish you the best in conquering this hideous affliction

@ggr a doctor (urologist?) who is unable to perform a cystoscopy is in no position to criticize their patient re hygiene. At the very least the doc should have found a colleague who could perform the job. How is your daughter doing now? Has she been able to get a 2nd opinion?

So sorry to hear about your daughter's health problems. I have been a member of the MAYO CONNECT over the years and have shared my numerous health problems. I am a 69 yr old, wife, mother and grandmother.
My urinary problem began in 2/2023, recurrent UTI's, abdominal cramping, was seen by an URO/gynecologist, diagnosed with urethral hypermobility, instability of the urethra, mixed incontinence, all probably attributed to having a hysterectomy 25 years ago, UTI's persisted, about 6 or 7 occurrences during the next year. Was see by a 30 yr veteran Urologist, (Conducted, FISH Analysis - Bladder Cancer genetic screening - Result NEGATIVE)
CT Urogram - (Result: Extrarenal pelvis of kidneys, no hydronephrosis, Bladder clear) MRI - Result Small incarcerated intestine, and internal hernia. Urologist (12/10/24) scheduled with general anesthesia sedation; a Cystoscopy, Urethral dilation and bilateral pyelograms. Urethral dilation performed, Chronic moderate hydroureteronephrosis with no evidence of obstruction. Patient exhibiting intermittent right flank pain and right groin pain. Urologist reported in operative report urethral stenosis my recur. Also stated I must see a general surgeon because I would need a hernia repair.
Had major abdominal surgery on 2/6/25: Laparoscopic right salpingo-oophorectomy. (ovary and fallopian tube removed), laparoscopic liver wedge biopsy, laparoscopic partial omentectomy; (ovary attached to omentum wall), Laparoscopic small-bowel mesenteric pannicullitis, small intestine attached to peritineum wall, open repair of incarcerated ventral hernia, laparoscopic lysis of adhesions, and scar tissue.
While recovering from this surgery, was continuing to experience, groin pain and bladder pain.
Went back to the urologist, he stated that the urethral stricture had returned and in his office with no sedation or pain control, or cystoscopy to view the rethral canal, just went in blind with a metal urethral sound instrument to clear a stricture even though was never confirmed with a dye scan or cystoscopy. For 10 weeks following this horrendous procedure, ( which probably should never have been done) suffered for 10 weeks with horrible bladder, urethral, groin and abdominal pain, his office would not give me an appointment because his nurse practitioner was out on maternity leave. I went to a walk in clinic saw a physician, conducted an urinanalysis and sent out for culture (Result: no UTI) the physician stated never have another urethral dilation ever again. 2 weeks later went to my primary care physician had a urine analysis and sent out for urine culture, came back with Enteroccoccus faecalis 49,000 CFU/ml infection, attributed to contaminated surgical instrument. PCP stated not to every have another urethral dilation. Also went back to my general surgeon, he ordered a transvaginal ultrasound and abdominal ultrasound, (REsult: NORMAL), He stated no more urethral and bladder procedures. Its been 3 1/2 months since the urethral trauma and injury and things have finaly settled down. I use a teaspoon of powdered D-Mannose in a glass of water 2 x's a week to keep UTI's at by. I drink water all day. I flush my kidneys very well, urine is pale yellow, no recurrence of UTI. I also started using Estraidol .01% in the vagina, 2 x's a week. Urethral pain has subsided. groin pain has dissipated and I am finally feeling better. This all really happened to me. Don't know what life has in store for me down the rode but I tell everyone keep searching, If you are not happy with your doctors find new ones. (Yes I now go to a female urologist, female gynecologist, female PCP) READ, READ, READ, You are your Best Advocate. and pray to your higher power, what ever faith you follow.
