Has Anyone Injected less than Standard Dose Tymlos w/Good Results
I began injecting Tymlos in late September of 2025 after bone scan reading of -3.2 in my forearm. Hips were replaced and spine has lots of sports injuries so neither of those sites were valid. I immediately had the usual side effects of headaches and bone pain, plus fatigue. Ugh. I was also concerned that at 105 lbs., the standard dose was excessive for me. I cut back to 70 mcg and even skipped several doses when I felt particularly awful. Took supplements, increased protein and started weights early November. Then November 16 had blood drawn for P1NP. This wasn't a true baseline as was taking Tymlos for 6 weeks at that point, so reading of 55 was likely higher than true baseline from late September. I have stayed on 70mcg and January 15 blood work showed P1NP increase to 97! Query whether others are seeing strong response despite less than standard amount of 80 mcg? Oddly, I have not increased muscle strength or volume and feel fatigued and sore after workouts at low weights. 2 hour workouts 3 times weekly.

wilomena, many are reporting good results with Tymlos at lower doses. Your P1NP show that you are gaining bone integrity. The trouble is that it will be difficult to measure in your forearm. And your forearm may indicate bone density loss even though the bone will be stronger. It is common in the forearm for dexa to indicate density loss and you may have to look at TBS and fracture risk to appreciate how Tymlos is protecting your bones.
My teeth are also a dentists dream.
We are not recommended to have dental work with the use of antiresorptives because the bone they preserve is less vascular, more brittle and lacks the ability to heal itself. The antiresorptives all have a warning about osteonecrosis of the jaw. The abundance of bacteria in the mouth can cause necrosis if introduced into the jaw bone. And it is very difficult to heal. Forteo and Tymlos are useful because they increase the bone turnover that removes and replaces damaged bone.
I find it wise to take the lower dose and to take a day off either Tymlos or Forteo
I also suspect that your forearm isn't your biggest osteoporosis risk. It is giving you hints about the thoracic which we never measure because the ribs are in the way.
@gently All very meaningful comments but with bilateral hip replacements and a spine that is compromised with injuries and other degenerative loss, from cervical down to lumbar, all that was left was the forearm and the forearm that they diagnosed has previously experienced a torn rotator cuff in the shoulder and a torn bicep, which has never been repaired. So that arm is of course compromised as well. I’ve had a very active lifetime beating up my body. Lol.
I am scheduled to have an REMS ultrasound, which will investigate the health of my bones and I plan to ensure they look at both my spine as well as my hips. I understand an RMS ultrasound can do so despite hip replacement hardware. Hopefully that will give me some insight into the bone health itself. I also had a recent NTX score of 22. I am told that is a good number. I believe I previously mentioned that not only am I taking a reduced dose of Tymlos at 70 MCG, but I have also skipped my dose periodically. In the four months since starting Tymlos, I have probably missed half a dozen injections if not slightly more. I am not recommending this to anyone because each of us reacts differently to Tymlos and I have now brought my body weight up from 105 to 110 Pounds Which I had been trying to do for quite some time. It is likely the increased protein I added to my diet, but may also be attributable to bone formation. I am hopeful for that. As for the thoracic spine, I suffered a muscle injury a year ago after lifting a not so heavy weight. I recently had x-rays that did not show evidence of a significant compression fracture in the thoracic spine. I’m hoping the REMS ultrasound will also investigate that area, ribs not withstanding.
