Foot osteoporosis

Posted by kars @kars, Jan 31 9:30am

The top of my foot has been giving me a lot of pain recently. It is an aching pain and I have trouble walking. I am even having trouble finding shoes that are comfortable enough to wear. Anyone out there having anything similar? Any input would be greatly appreciated. Thanks

Hi @kars, I'm just wondering how you're getting on, and if you found out anymore about what is/was causing your foot pain.

I haven’t gone to the dr. About it but it hasn’t bothered me much lately. It kinda comes and goes, which leads me to believe that it is some sort of arthritis. It is a chronic thing, so I know that I will most likely be bothered with it a lot. Thanks for asking. 😊

Yes, same recent problem. MRI showed tendonitis, drop in hormones can increase this.

Hmmm good chance that is my problem also.

What did the doctor say for you to do? Hope you are doing better.

I have struggled with two stress fractures that took a long time to heal. Just curious, what kind of insoles do you use? I have found that a topo athletic shoe works the best for me as it is wide toebox. I have custom-made orthotics that I don’t feel are super helpful. Thank you so much.

I will check out these shoes. I also need a wide toebox. Thanks for replying.

The one I have is based on pulsed electromagnetic field (PEMF). I wear it three hours a day minimum. It's supposed to promote healing at the fracture site. I cannot feel it. It doesn't shake my foot. The stimulator prescribed to me is made by Orthofix.

Thanks so much @mev123. This device looks interesting and I sure hope that it helps your metatarsal heal. I will ask my doctor about it at the next appointment if my metatarsal still has not healed.

But, too much testosterone can cause tendinitis also

I have osteoporosis and I've been taking alendronate for four years. I have a stress fracture at the base of the second metatarsal. It's been a year since it occurred and it's not healing. Officially, it's been diagnosed as a non-union fracture. I talked to my podiatrist about why my fracture isn't healing. He said it's likely a combination of osteoporosis, my age (I'm 71), and alendronate. I'm now on a bone growth stimulator (week 7). I guess I'll know if it's working in a few more months but I don't feel or see a change in my foot.

@mev123, Did you wear a boot when you first hurt your metatarsal? I didn't because the podiatrist didn't know if I had a stress fracture or a neuroma, because it didn't show on the xray. I asked for an MRI but that took 4 weeks to get results after waiting for an appointment. Now that my foot isn't healing, the podiatrist wants me to wear a boot. The problem is the weight of the boot on my metatarsal makes it hurt. I am 73, petite and on Tymlos. The only thing the podiatrist said when I asked why this bone wasn't healing like the others, was that it is weight bearing. Have you been walking on your foot all this time? I was thinking about one of those knee scooters.

Hi Sue -
I did wear a boot for the first 8 weeks. I've had a fracture of a different metatarsal in this foot about 12 years ago, so I was over-confident and I didn't think I needed to see a doctor. I thought, "I'll just wear my boot like I did last time and it will heal." After 8 weeks, my foot was still in considerable pain. That's when I went to see a podiatrist. The podiatrist gave me a rigid shoe to wear which is much more comfortable than the boot. It is very rigid. I can't bend my foot in it. I wear the shoe when I know I will be walking a lot. I don't wear it around my house, but I probably should. I have some Hoka sandals I wear around the house and I feel no pain in them.

Here's a link to the rigid shoe on Amazon:

I found the shoe did hurt when I first started wearing it, but I quickly adapted to it. Heck, at my age (71), any change in a shoe hurts for a while.

I see the podiatrist every month. He x-rays my foot. Initially, the x-rays showed some partial healing but over time it was not changing. I've gotten several CT scans which provides a better picture of what's going on with healing around the fracture site. The last CT scan showed I'm losing bone around the fracture so there are no new calluses being formed. That's when the doc prescribed the bone growth stimulator. I'm not a good candidate for surgery given my age and osteoporosis. I don't want surgery.

I hope this isn't too long winded and I hope it helps. Best of luck with your healing. This is not the way we want to spend the final stage of life.

Thanks @mev123 for your helpful comments. The hard shoe that you are wearing is like the one that I have to wear now. I don't like wearing it as it hurts other parts of my foot. Like you, I only wear it when I go out. I like to wear oofos slides at home and they don't hurt. I am seeing a podiatrist and an orthopedist and it doesn't help. The fracture didn't show up originally on an xray. It took 4 weeks to get the MRI diagnosis when I was finally instructed to wear a boot. I should have worn one right away. Probably, seeing a doctor wouldn't have made any difference in your care.

I think my fracture is in about the same place as yours at the end of the second metatarsal. My second and third toes are inflamed, raised up and painful. The orthopedist told me to push them down as they might get stuck that way, but it hurts horribly to even touch them. I'm afraid that they may already be stuck.

It is upsetting that the medicine that is supposed to help osteoporosis may be what interferes with fracture healing.

Hi Sue -
My podiatrist told me my toes hurt because my foot is swollen. Wrapping my foot with KT tape helps with the pain in my toes. Because it's been about a year since I fractured my foot, I have very little pain in my toes except at night when the swelling in my foot increases. I can bend them now, but I could not when the injury first happened.

My fracture is at the base of the second metatarsal which is closer to the ankle. I read it's more often found in ballerinas of which I am definitely not. 🙂 I have abnormal bones from osteoporosis so that's primarily why I'm dealing with this.

I am resigned that I will likely have to wear the rigid post-op shoe for the rest of my life. At least I can still walk. Thinking happy thoughts.
