Fat pad atrophy and arthritis in both feet: What meds help with pain?
Hi, I have fat pad atrophy and arthritis in both feet, and am in pain 24/7. What meds would help? (I'm on Clonazepam and Zoloft because all this pain and worry caused me SEVERE panic attacks, so it can't interact with those). I can't take NSAID's, but I do use a topical one (Diclofenac Sodium Gel 1%), with little to no improvement so far. Maybe a higher dose prescription one? I bought insoles the doctor recommended, (just from Amazon, not the custom ones), I've bought over a dozen pairs of shoes and haven't found any that help. I can't go barefoot, even in the shower. I've tried Epsom salts, castor oil and cayenne, physical therapy, (although when I did that, they thought the problem was neuropathy, which was later ruled out with an EMG), and I still do the exercises that don't increase the pain. The doctor said to stay off my feet as much as possible, and use a wheelchair if I have to go any distance. Sitting all day isn't healthy, and my husband has to do everything. Also, it's hard to differentiate whether most of the pain is from the fat pad atrophy or the arthritis. Does any Mayo clinic have experience with any type of fat pad repair? I know this is long. Thank you for reading to the end, and if you have answers or suggestions to any of my questions, I'd really appreciate it. Thank you!

I wear Hoka running shoes because they have the most cushion. There are different models so you should be fitted at a store. I also wear custom fitted orthotics from a podiatrist. It has a small bump just below the arch. This really helped the pain in the ball of my foot. Hope this info helps you.
Thank you. I tried Hoka, but maybe just not the right ones. When you got you orthotics, were they hard to get used to? I've heard a lot of people say they spent $400-600 and couldn't wear them. So you take the insole out of the Hoka ones to use the orthotics? Do you have to go up a size in shoes to accommodate the insoles or are they only about as thick as store-bought ones? Do you have the same as me....fat pad atrophy and arthritis or do you have metatarsal pain? Sorry for all the questions, and thank you again for responding.
i just bought a pair of adidas on sale tying laces might be hard. hopeful.
I did spend $600 on orthotics and they did not help so I went back to the podiatrist. The orthotics were redone with the small soft bump which made all the difference. A podiatrist would know what I mean. I had pain in the ball of my foot, as well as metatarsal arthritis and neuropathy. Hope this helps as foot pain is terrible.
I forgot to tell you that I take the Hoka insoles out and put in my orthotics in my shoes
