Fast Rising PSA post mri showing one pi-rads4

healthy and fit 67yr male, had an mri in december (after 5.2 psa). mri came back showing one lesion in medial periph zone, Pi-Rads 4. Urologist said "your mri looks fine, no biopsy needed yet" WHAT!? he suggested we get another psa test in a month or 2 and go from there. last weeks psa was 9.4 🙁
seeing a new uro this week, gonna request another mri.
any other advise/support would be GREATLY appreciated 🙂

If have a PSE and it says you cancer do you really need to do a biopsy can't you just move forward with treament?

If you decide to do a prostatectomy, then you could move forward, though I would ask for a PSMA pet test or an MRI. The MRI can give you information on how extensive the tumors are, Whether there is noticeable spread outside the prostate as can the PSMA pet.

With the right MRI you can get a Gleason score estimate not with a pet test.
