Early ADPKD with family history of brain aneurysm – advice on monitori

Posted by abdullah07 @abdullah07, 3 days ago

I am a 20-year-old male diagnosed with early autosomal dominant polycystic kidney disease (ADPKD). Imaging revealed six small cortical cysts in each kidney, with no stones, solid masses, or hydronephrosis. Kidney function is preserved, with normal creatinine and eGFR. I have persistent hypertension averaging 145/95 mmHg, currently treated with Atacand Plus 8 mg daily. Two episodes of gross hematuria occurred in the past two months, both self-limited. LDL cholesterol is elevated at 4.57 mmol/L. There is no family history of ADPKD, but hypertension is present on the paternal side. I seek expert advice regarding optimal management of early ADPKD with preserved renal function — specifically blood pressure and lipid targets, recommended follow-up imaging intervals, and preventive strategies to slow cyst progression.
