Dental work while on Reclast?

Posted by sebutler @sebutler, Feb 6 11:01am

Has anyone had dental work done while on Reclast? Any problems, particularly if the dental work involved an extraction or an implant? A published survey found that cases of jaw necrosis occurred two or more years after beginning Reclast, but six years or more for patients on Fosamax.

I have recently inquired about having some orthodontic work done and was told that studies show very poor result for patients who had reclast injections, and was discouraged from pursuing this. I would encourage you to provide your health history to any oral surgeon you are dealing with and make sure that they are aware and informed about reclast before having any dental work.

Thank you for your advice!

I have been taking Tymlos and have had really good results. I have had 22 back surgeries and since taking the Tymlos the results have been a lot better.

I have been there. I think orthodontic work is perfectly fine. I know from bone doc and maxofacial surgeon that one should NOT have dental extractions 5 years after Reclast, also not certain root canals those which go high up in the bone. I abided by that. It ruined my smile. But I did not want Osteonecrosis. The surgeon at a major university hospital vividly gave me a portrait of what could happen re osteonecrosis.

Five years after Reclast!! That's a long time! I hope my teeth can hold out!

I’m wondering how is someone supposed to wait five years to have a tooth pulled or a root canal if it’s needed. It’s crazy! And the doctors tell you this without even blinking like it’s no big deal. I’m supposed to go on Reclast after Tymlos. I’m terrified. But I don’t what my options are other than a biphosphonate. I’d be grateful for any suggestions. Thanks.

I'm in the same boat. Because the incidence of jaw necrosis occurs 6 or more years after start of Fosamax (see my post above), I was thinking of taking that after Tymlos. But my doctor said it is not absorbed as well as Reclast.

I just started Reclast (actually Zometa which is twice a year to help horrible osteoporois after 2 years on Letrzole and perhaps reduce potential Breast Cancer mets) this past October with another due in April at Dana Farber. I'm 59 yrs old and yesterday my dentist expressed concern as cracks in old fillings (prob 30 yrs old) are putting me at high risk if tooth cracks as he knows I'm on Zometa infusions. He is a prosthodontist and feels very comfortable with me having a crown, even a root canal (which scares) but obviously extraction not recommended. Even with "babying" my teeth (nothing crunchy etc) I am not so worried tooth will crack - have any of you had a crown while on Reclast. My oncologist is thinking 3 years for the Reclast/Zometa? xoxo

Thx So much for sharing! I’m 60 yrs -had first Zometa(it’s Reclast-slightly different dosing as along with osteoporosis I’m hoping to minimize risk of breast cancer Mets) in October-next one this April 2025-a few old fillings in back teeth (4 of them) now showing cracks-prosthodontic doc recommended crowns (of course not at once) and he feels comfortable but I worry he could crack tooth but I also can’t baby my teeth more than I am as recommended 3 yrs Zometa/Reclast to help as on AI’s for Breast Cancer for 8-10 yrs and I'm only two years in. I’m going to start with two crowns but honestly petrified. What did your oral surgeon share about “worst case” if you don’t mind sharing ? Xoxo

I had successful refilling of old fillings after having my first reclast injection with no problems. It is orthodontic work that I was told would have little success as the reclast makes your teeth more resistant to movement.
