Chronic pain after peroneal tendon surgery

I had surgery in April 2025 to tighten my superior peroneal retinaculum and remove a low-lying muscle belly. Since the surgery, I have had persistent pain, including a tight, painful, pulling sensation from where the retinaculum is. There are permanent, synthetic sutures (2 Arthrex knotless Fibertak suture anchors) in my retinaculum. I was very diligent about my PT and increased my range of motion - for a couple months I felt that my pain was getting a lot better. However, most of the last year the pain has been getting worse and worse. I tried PRP and am now trying shock wave. I'm really struggling to even walk around the house without pain, and I am an otherwise healthy, active, fairly young person.

I'm curious if my suture anchors are causing irritation, or if they are too tight. The MRI showed a lot of scar tissue, but nothing else. I'm considering a revision surgery and meeting with another surgeon soon - what should I ask? Has anyone else had similar post-operative pain and had it resolve?

The scar tissue may be the culprit. And you said you had PT. Consider going back to PT after your surgeon clears you again and consider a second opinion.
