Chronic EBV with abnormal labs

Posted by jdee3 @jdee3, 1 day ago

PCP tested me 1.5 years ago because she said I always looked tired. All EBV antibody titers positive. Have repeated tests multiple times and always the same. Recently tested whole blood dna which was positive. Will list most recent labs. 60 y/o not immunosuppressed.
EBV Ab VCA, IgM
>160.0
Reference Interval: 0.0-35.9, Unit: U/mL

EBV Ab VCA, IgG
Result Notes
55.2 U/mL
Normal 0-17.9

EBV Nuclear Antigen Ab,
Result Notes
IgG>600.0
Reference Interval: 0.0-17.9, Unit: U/mL

EBV Early Antigen Ab, IgG 17.8 U/mL
Nl 0-8.9

EBV PCR Quant (Whole Blood)
Result Notes
143 copies/ml
Reference Interval: Negative, Unit: copies/mL

log10 EBV DNA Qn PCR
2.155 Unit: log10copy/mL
These were all done at labcorp.
PCP tested me for Lyme multiple times as well as bartonella. Was positive with IGM ab for two strains of bartonella at Igenix but all tests done recent for it at Lab Corp negative. Had mild elevation of SSB. Ana negative.
Symptoms of fatigue seemed to stay after Covid which I had twice. Long term joint pain possible seronegative RA or PSA dx. Stopped tremfya when I got Covid 5 years ago and never restarted due to concerns with Covid and then dx with ebv. Saw ID a few weeks ago but she did not seem very knowledgeable about ebv and I’m sure she thought my dna would be negative. My pcp had vaccinated me for pneumococcal pneumonia prevnar 13 2 years ago recent check of antibodies show I formed them to only one of the 13 so did not mount normal immune response.
Routine IgG levels normal. General labs all normal.
Who should I see? Feel very frustrated at the lack of knowledge most physicians have including ID. Is there someone at Mayo that specializes in EBV and possible immunodeficiency? Closest to Florida location but willing to travel wherever I need to go.
