Can't decide which drug to take.

First time on a forum of any kind. Unsure how to do it. I'm 85 and need to go on something for OP. Evenity has been recommended followed by Prolia. I was on Fosamax at 63 which made me feel really sick and then threw up. Switched to Actonel and stayed on it for 7 years, got off at 70 because of reflux problems, then told to stay off it because of fracture risk for long term use. I'd moved so had new doc. Breast cancer at 76, lumpectomy w radiation, stopped HRT. DEXA recently at 85 showed "extreme" OP and current PC wants me on Something! I've refused for years because of side effects. Also during 2023 my husband of 38 years was dying and in hospice for 18 months at home, died in February 2024, so my mind was diverted there. Have developed horrible GERD, taken Protonix w pepcid, trying to taper off, but very hard to do, necessary because it eats yr bones.
After recent massage, I developed sharper pain than usual that hasn't gone away, my PC won't be surprised if I have a compression fracture in my spine. I've never had one so don't understand where it could really be. I've had 2 TIA's in 2023, so don't think Evenity is right, bisphosphonates wd make me sick, afraid of Evista, that leaves the two parathyroid meds Forteo and Tymlos which sound Awful! Any advice on what to pick wd be appreciated. I'm pretty scared!

I started Tymlos 3 weeks ago. My OP is -4.8 spine.
Tymlos (it’s newer) builds bone in most people quickly relatively speaking. 18-24 months. The injections are painless and easy. Although my husband does them I think I’m getting enough confidence to try myself. But he makes a nice blanket and pillow for me, lets me relax watch an old movie, does the injection around 8pm not many side effects if you do the things you need to. Hydrate, no calcium just before or after injections but everyone is different. You might ask about following up with Prolia. Not a doctor but my husband does have a medical background…some have suggested looking Strontium. Some are pretty religious about it so tread carefully…❤️

Although my husband does them I think I’m getting enough confidence to try myself. But he makes a nice blanket and pillow for me, lets me relax watch an old movie, does the injection around 8pm not many side effects if you do the things you need to. Hydrate, no calcium just before or after injections but everyone is different. You might ask about following up with Prolia. Not a doctor but my husband does have a medical background…some have suggested looking Strontium. Some are pretty religious about it so tread carefully…❤️

@osteopatient2026 Do you know algaecal product, offers calcium one bottle and strontium in second bottle? Many on Tymlos say blood pressure drop after shot can be bad w vertigo and rapid heartbeat. I hear the company is good about Zooming other patients to help you get used to using. I’m a widow now so no one at home to help, wd have to do it myself, chicken about that, maybe cd learn. Live way out in country.

So sorry for all you have been through..and I get why taking more strong meds is not desirable….no answers here, but check out our spine, X-ray or CT…if a fracture, maybe docs can help you stabilize it to prevent more problems…meanwhile, be very careful not to fall…you are not alone here !

@osteopatient2026 Very grateful for your kind encouraging reply. Yes, all are different, and I seem to be side-effect prone but one never knows. I'll worry about follow-up once the two years are up, after all I'll be 86 soon. I wondered if early evening was best time, so thanks for that, I just hope I can learn to do shots myself.

@nycmusic I'm so grateful to know I'm not alone. Most where I am just do what their docs tell them but after 2 TIA's I want to avoid the Evenity I've been offered. I think in Europe people with any history of stroke incl TIA's are now allowed to use it, so Tymlos may be my best choice.

Yes they were great with help on the first injection.
My BP is 90/60 so I was very worried about that as well. But didn’t experience anything dramatic.
It did go down but only briefly for 10-15 minutes. Some people report it is up to an hour or so. But the numbers reported are the limits of what has been reported. Most (90-95%) I read said don’t have that extreme.

Next the fast HR …yes it is stimulating your Parathyroid so a “fight or flight response” on the sympathetic nervous system …my HR is usually around 55-60 (I was a runner) it went up to 90-95 for about 20-30 minutes then began coming down. Deep breathing helped with that. Relaxing. You can easily increase your heart rate just with the anxiety…I certainly had a boat load of that.
Hydrate for 3 days and everyday. During some electrolytes (not just water …that actually just flushes out your system.
You can drink a glass of water 1/2 before and some after 15-20 minutes or more. This will help with the dizziness or vertigo which I also have major issues with.
Don’t consume calcium 1 1/2 hour before or 4 hours after you injections. That helps prevent or minimize the temporary nausea you might feel a few days in.
All this was just the few weeks till my body adjusts.
For me so far so good.

For most early evening is best… 8 pm works for me I go to bed around 9 -9:30 so allow about that time and have a piece of toast and jam 1/2 hour or more after. That really helps.

Also watch the “too much calcium” …some of these meds require extra calcium …some not too much Tymlos can cause too much of a calcium dump into your system causing Hyper rather than Hypo calcium…sometimes kidney stones.
But if you are hydrated properly and not over doing the calcium you most likely will be fine as 90-95% are. You should get some blood tests done prior for baseline.
My calcium was right in the middle of normal 9.5 and is now 9.7. Will keep an eye on it.
Remember almost all things you put into your body even so called “natural supplements” can have severe side effects and adverse reactions. Aspirin kills over 40 people annually and 20,000 overdoses! So far there hasn’t been any directly associated fatalities from Tymlos.
My Tscores were so bad (-4.8 and -5.1) I chose to go this course many others choose other paths.
I’m only 65 but I’ve seen many people try the “natural way” to beat cancer some did most didn’t. There are plenty of snake oil sales people on both sides. I know it probably can’t be taken any other way but I’m not pro drug …this was just my choice (again my husband has an extensive medical background so that did help but he is the first to claim how bad the dictate these days…take the time find a good one.

Your info so helpful. My Tscores were very bad, -4's and maybe beyond, I'd have to look them up again. You've given me hope. My memory not so hot at 85. Had a CBC lately and calcium was 9.3 which that cancer doc said fine. I'm coping with a probable compression fracture today, will see my FNP Wednesday this week, probably send for xray to determine what. I don't like these drugs but I know I need one and Tymlos seems most suitable for me, being allergic to bisphosphonates and after 2 TIA's. I'm just afraid of injection daily. I'm 5'2" and 101 lbs, with an elder's pot but appendix scar of old, so not a lot of room. I bruise easily and heal slowly. Such a drag! I really need a good endocrinologist but they're over an hour away and hard to get appts with and all have different opinions, so I think I'm stuck w my own judgment on drug choice. I've heard the Tymlos company is helpful w zoom appts and pretty good advice.
