Can a dry orgasm after prostatectomy still be pleasurable?

Hi all,
I had my prostate removed and was told that my orgasm would be dry due to the removal of the seminal vesicles. Is it still pleasurable to have sex without that feeling of fullness when you are about to orgasm?

My experience...

Yes.

Sex is different after surgery albeit experiences may vary.

My surgery was nerve sparing

Erections took about a year to return to "normal."

Initially I had some urine leakage, went away with time.

Achieving an erection required greater physical and mental stimulation

I do still have some cowpers fluid

Orgasms were dry but intense.

I have been on 5mg Cialis daily since my surgery.

My PRT consisted of use it or lose it, aka, masturbation.

Again, my experience.

I get dry orgasms, but I still make Cowper's fluid and ejaculate that instead of Semen. My Orgasms are pleasurable and intense. 🙂

@jayhall What is Cowper’s fluid? Is it clear cum? I have that when pulling it out of her vagina.

You guys are giving me hope! I haven't had an adequate erection or climax since starting ADT 17 months ago. I go off med at the end of June and am crossing my fingers (and uncrossing my legs!) that the ability will return. I've been taking 5mg Cialis for blood flow and using a vacuum device after showers to stretch the tissue. I tried Trimix but there was either no erection or one with lots of pain, so I discontinued and haven't experimented otherwise. If not, my wife and I had a consult regarding an implant but I'm not sure she's on board for that as the alternative, yet. I'll know better in September, after some recovery time.

RARP Oct 25. Getting good solid erections, but can't reach orgasm. Still hammering away. LOL. Got on daily low dose cialis and can't really tell a difference. Urologist recommended adding viagra. Haven't tried it yet. I've always been a spontaneous type, and haven't worked in the planning aspect of 1 hr onset.

@folmajt Hello I also get dry orgasm's but in thr beginning My penis gets a lot of pre cum if excited. Better than using lubricant.for intercourse.

I do but I still leak. A urologist in NYC, Dr. Eid, that I consulted with regarding a penile implant told me a lot of guys will leak prior to sex just thinking about it. Same with me. If my lady gives me a hug, a little squeeze down there or talks sexy, I leak some.

I was married when I had my RP, triple therapy and radiation. Great sex life before the cancer and treatments. Not so much afterwards. I had some incontinence (really started months after my pelvic radiation, not surgery) but also the urine ejaculation with climax. I don't think she liked that. Had to wear a condom. Ended up with a divorce (other issues as well).

Met a wonderful, understanding woman. She doesn't care at all about the leakage during sex-"just another bodily fluid, what's the big deal?". Truth is, the climaturia (urine ejaculation) feels good and kind of like ejaculating semen. No condoms for intercourse or oral. Wonderful.

I'm scheduled for a penile implant in a month. The oral meds never worked after surgery 5 years ago. (I had complete ED). Vacuum device with rings weren't acceptable. I have been using the Trimix and now Quadmix for several years. Worked great in the beginning but increasingly ineffective. I have read extensively about the penile implant and satisfaction rate is >90%. Looking forward to more spontaneous sex without needles. My partner is the one that suggested it. Told her "careful what you wish for". Can stay erect as long as you want. I'll be chasing her around the house! Got to laugh sometimes with everything we have been through.

Thought I would mention if it is helpful to anyone preparing for RP that most of my orgasms following surgery have been more pleasurable than prior to surgery even when flacid. They are longer and more intense. Hope this gives hope !!!

Yes, I have experienced the "shoot pee'. The medical establishment calls it climaturia. This was never discussed with me as a potential "complication" of RP. Have you done or know any way to reduce the chance/amount?

Sixty year old with nerve-sparing RALP in August 2025. Like many others, my orgasms are more intense than before. I also produce much more “pre cum” due to over active Cowper’s glands - an effect of the nerve sparing, I assume.

That being said, it took me a while to come to terms with the loss of ejaculation. My fixed mindset had me believing that was the goal of intimacy. I just retired from a career as a therapist, so I was able to overcome that quickly, but if it’s an issue for you, talking to someone might help.

I’m on daily Cialis and Viagra as needed with frequent penile rehab (aka masturbation) to keep the blood flowing and to encourage “muscle memory”. I’m also fortunate to have a wife who is still as interested in sex as before and has been willing to adjust to the new normal. Again, if that is a barrier, a few sessions with a sex therapist might be a consideration to help redefine “pleasurable”.
