You need to speak to a Surgeon (urologist who does surgery) and a radiation oncologist. Both of them have about the same results though surgery may make more sense if you’re young (below 70).
Just going to a cancer clinic may be a mistake. You want to go to a place that specializes in prostate cancer? Mayo Clinic is one of them. Prostate cancer is not easily handled by just a clinic, You want a Genito Urinary Oncologist Handling your case if it is complex, A place like Mayo has top notch oncologist that specialize in prostate cancer.
You can do a telehealth from most centers of excellence. I did 5 telehealth's with radiation oncologists who were either at centers of excellence or those trained at centers of excellence. I was treated by one of those at the Orlando health cancer center but per @jeffmarc's suggestion, Mayo is a great choice and they do telehealth appointments.

Can pass on my experience with Mayo Clinic Jacksonville as I have been a patient there since 2006. The term Clinic can be misleading. Mayo and Cleveland Clinic are referred to as clinics. They are far more than a limited clinic having hospitals and specialties, treatments, and every medical and mental health specialty available.
You will get outstanding care there. The doctors and surgeons are top notched and everything medically that can be done for you, your specific prostate cancer, and surgery, is what they specialize in.
Dr. Pathak got neg margins w ECE for me, and he took 12 lymph nodes, so I def recommend him for Mayo Jax. Apparently Dr Pak is one of the best too. But Mayo can be a bit of a bureaucracy and hard to get timely appointments, particularly MRIs. Moffitt in Tampa is prob very similar and way less of a haul for you.
When I was researching options I consulted with both a surgeon and an RO at Moffitt and was very impressed with the whole operation. I decided to go to someplace closer to my home in South Florida, but if I lived in St Pete they would be high on my list.
....Center of Excellence, Technology, Skill of practitioner on that technology.
(Don't forget the view from 30,000 feet) such as the CONTEXT of the options. The PCRI.org videos on You-Tube will provide context. However get the most recent videos. The same Gleason score numbers alone 5 years ago have have less currency now with the advent of PSMA PETCT scans which detect (macro)metastasis (>2-5mm)].
MAYO in Jacksonville, Silvestre Ctr Univ of Miami and Moffitt are designated as centers of cancer excellence by NCI.
Dr. Pathak did my surgery too. I was 4+3, tertiery 5, negative margins, neg SVI, neg, 13 nodes, did show cribriform and PNI. I've been < .1 for 22 months now. Are you using Mayo for your salvage treatment. Who prescribed your ADT/ARI? Did Pathak refer you to a RO or MO? Just curious about the process. They have me seeing a NP for my psa checks. I go back in October and hope it's still < .1
Thanks, Jim
Others have mentioned that my comments about a “cancer clinic” May not clearly define what the word clinic implies. Yes, Mayo is the Mayo clinic but they are a COE. You want to make sure when you’re going to a cancer clinic That they are a center of excellence not just a “cancer clinic” Prostate cancer requires a certain amount of expertise in that area.
I definitely should’ve been more clear in my recommendation.
UF Health Cancer Center in Gainesville is one also.
