At home care after discharge

Hello! My FIL is in the hospital at Mayo Phoenix and we are living out of state. We would like to speak to a social worker about after-care options but cannot find any information. Can someone point me in the right direction to get started? Thanks!!

@absolutra1, so wise of you to plan for after care. My understanding is that you should ask your father-in-law's RN Coordinator to put in the request for a social work visit. He can do this while in the hospital by simply asking the nurse on duty to speak with his RN coordinator or the next time the doctor makes their rounds. A family member should also be able to make the request.
