Anyone's pNET Reoccurrence Morphed from Grade 1-2 to Grade 3 ?
So My original pnet was on tail of pancreas and removed it was Deemed Well Differentiated, Grade1 initially on histology and then ended up being stage 2, after ki 67 came back as 6.7 (but mitotic rate was < .02 ) Krenning score was 3(vivid uptake on dotatate)
Now new suspected reoccurrence of 1.2cm close to resected margin and dotatate scan shows zero uptake! This is truly scary to me. Has anyone had a well differentiated, Grade 1 or 2 that initially was dotatate vivid (krenning3) reoccur as completely dotatate negative?
I am really concerned because this closes a lot of treatment doors… and the ones that remain open are chemotherapy drugs.
Anyone have a SECOND surgical distal pancrectomy? I worry a second surgery in this area will be more complicated. But I will seek this option if it is available to me.
I have no distant metastasis currently and have 3 years after initial tumor removal of no distant metastasis. So at least I have that going for me. See my oncologist surgeon on Tuesday.

@frogsong, no distant metastasis is good. But I can understand your concerns about the staging being change to grade 3 and how this changes the treatment options. How did your appointment go with the oncologist? What are the next steps?
