ANCA positive

Posted by crocodile @crocodile, 1 day ago

Hi everyone. I test positive for c-ANCA but have no symptoms of anything. The level is 6.9 and titre 20 which I don't know if it is high or low. Just wondering if this is an indicator of something brewing or don't worry about it.
Thanks.

@crocodile Welcome to Mayo Clinic Connect! You are wondering if the level 6.9 and titer 20 mean something. The doctor, who asked you to have the bloodwork done, probably has all the answers for you. Have you gotten an appointment? I’m sure other members will some along and tell you their experience. These can be very good because they will let you know what to expect. And you’ve come to the right place! The members will give you very good/personnel experience. Which is better than any book!

Thank you for your reply. The doctor doesn't want to do anything unless some symptoms of anything appear. So far they don't appear to. I'm just wondering if it may be an early sign of something brewing a bit later on. I have routine blood tests for my T2 diabetes every 3 to 6 months and the c-ANCA showed up and is always there. Started as 4.6 in 2018 but slowly going up. 6.4 at last test. Diabetes is well controlled.

It seems that you may be at risk for autoimmune vasculitis or inflammatory bowel disease. Are you working with a rheumatologist and vascular specialist? What are your current symptoms that had the doctor test you for this?

Thank you very much. I was seeing a immunologist but stopped when the levels were seen to be not getting any bigger and I don't have any symptoms. The tests were actually ordered by a neurologist due to some peripheral neuropathy. My diabetes has always been well managed so he was looking for other things that may cause this condition. Already tested negative for vasculitis and Wegener's. Neuropathy isn't really bad, just some slight numbness. Otherwise, I have no other symptoms. Was just looking to see if this positive ANCA may be something brewing and to see if the level I have ( 6.4 ) is high, low, medium or otherwise.

Reassuring that you have it for so long without developing a problem.

It does seem your body has some form of inflammation. May not be significant enough at this point to pinpoint disease.

I read that normal results for ANCA are titers < 1:20 and less than 1.4.
Adults [7]:
Qualitative: Negative
Quantitative: Less than 1.4 units/mL (less than 1.4 kilounits/L)

@crocodile
If your result is 6.4, this would be flagged as abnormal/positive and believe the higher the number, the higher inflammation due to the autoimmune attack on your system/cells due to the antibodies.

Have you ever seen a rheumatologist? You may want to follow up to check for autoimmune issues.

Thanks, I think you make a lot of sense. I'll revisit this with my GP and ask for a referral. Checking my blood test results, this has been with me since 2017. Still no symptoms yet. I should have written that inflammatory markers are also negative. I was seeing an immunologist rather than a rheumatologist. I understood that the roles are similar for this. I will discuss more deeply with my GP which way to go.
