Strange symptoms beginning 10 months ago mostly ENT type issues
I've always been a healthy person and until a hip replacement 3 years ago never had any health issues.
I'm a 66 year old male, 255 pounds 5' 11. Yes overweight but I eat well, and in the last year I became even more health conscious and eat healthy every day, lower cal and slowly loosing weight. I was 270 8 months ago. I do not smoke and I drink daily mostly wine, and try to keep it within recommended healthy limits.
Suddenly last summer I started feeling more tired than normal and kept getting reoccurring cold like symptoms that would last 3-5 days, would feel better, then it would come back again within a month. I do suffer from allergies, so i thought it was just a bad year, and put up with it.
Slowly it seemed to get worse and worse. After a Dr Visit he did mention to me that he had been watching my Immunoglobulin levels for several years. The tests always show Immunoglobulin A as high (351) and Immunoglobulin M as Low (35)
This is the first time I was aware of that. As months progressed I suddenly started seeing protein in my urine. It got quite bad at first. At the same time my episodes of cold like symptoms (no fever) seemed to get worse. At one point I also had ringing in my ears, and went to see an ENT who proscribed an antibiotic. It seemed to go away, but I always had a feeling that something was there, but he kept insiting everything looked good.
At this point a revolving door of symptoms developed. Very light cold like symptoms, with mild to severe tension like headaches, mild ringing Tinitus in ears, always more prevalent on the left side, and mild dizziness and light nausea. This would stick around for 2-3 days and go away. Coincidence or not, it often seemed as if the protein became worse.
Slowly things would get back to almost normal, energy levels come back, protein in urine would almost or completely disappear, and ringing in the ear would almost completely disappear.
Also, during this time I found it uncomfortable to lay on my side where my left ear area was pressed against the pillow, it would be slowly a dull pain to the point I'd have to roll over and sleep on my other side.
I finally got in to a kidney specialist who did an ultrasound and blood/urine work and found that I had a slightly elevated protein level that he said nothing to worry about, and we would check every 6 months. ultrasound showed normal kidney and bladder.
Protein could be transitory he said, and if it got worse he would do a biopsy.
About 2 months ago I suddenly had a spike in blood pressure while I was having another episode.
I always had hypertention and was in the 135 range but never to the point I needed meds. This particular occasion it was 190. Maybe because at this point Im getting worried about my health so not sure if related or not but I ended up at the emergency room.
They did an CT scan and bloodwork, results came back normal so they gave me a shot to reduce BP and told me to see my doctor.
I made an appointment to see my ENT again because I had the ringing in my ears again quite bad. He immediately put me on BP meds because I was 180 in his office.
He reviewed the CT scan and said it looked normal, but said that it looked like i had a small void behind my pituitary gland. He said it could be idiopathic intracranal hypertension.
We went over the symptoms and WOW I thought, thats everything I have!
He then sent me for an MRI and weeks later reviewed the results.
He said that they could see it, but were not sure it was big enough to be causing my symptoms or is the issue. I was devastated!
He suggested I see an Immunologist and a Neurosurgeon to make sure all bases are covered. It takes weeks and weeks to get appointments and no one seems to be putting the pieces together. Everyone seems to be looking at their particular focus and shuffling me from one specialty to the next with no results!
I've always been a big advocate of Mayo so I stated trying to get an appointment with some of the symptoms, but I keep getting rejected.
The last was with Neurology, where they asked for the reports and MRI and after 2 weeks they told me that my situation does not appear to meet their requirements for a visit.
So now here I am, feeling like I am just going in circles and appears to be getting worse ever so slowly over time and feeling like I am on a treadmill to a slow death.
My symptoms have completely changed my lifestyle, and thankfully I'm self employed because I dont think I could get up early to go to work with all my symptoms.
Summary of symptoms:
Reoccurring cold like symptoms 2 - 3 times a month (no fever)
Low energy levels lasting up to a week
Ringing in ears worsening with above symptoms (mostly left) dizziness/mild nausea
Protein in urine, sometimes goes away for days. Seems related to headaches
Had feeling of something draining/dripping in left ear area at times during peak issues
Blood pressure issues - not sure if related
Immunoglobulin A high (351) / Immunoglobulin M Low (35)
All other blood test in normal range
Protein in urine measured as minor around 300.
Waking up at night around 3:30 and difficult to go back to sleep. Related? Worry?
When at its worse, strange feeling of weakness and little shaking of hands to the point
that I wonder if I'll pass out. I get up, walk around and it passes in 15 - 30 min. This has happened about 5 times in 10 months always when symptoms are at worse.
ALL the above happened around the same period of time starting 10 months ago.
If anyone can help I would be forever grateful.
Rick

Dear @rickfl, after reading your post a few times, two things stick out:
1. You had no health symptoms until after your hip replacement.
2. When you took antibiotics, you felt better.
Can you have the implant checked to make sure it isn't causing systematic infection--maybe a surgical instrument/gauze was accidently left in the hip. Could you be allergic to the replacement part material? The fact the antibiotics made you feel better says a world. If the part is being rejected, could that cause infection? If you do have a systematic infection, maybe it could manifest in other bodily areas such as ear drainage.
It is just my humble opinion, that somehow the hip replacement has something to do with this. One last comment. If you take any OTC products for sleep, colds, allergies, etc., STOP. Many times, these products can make a person very ill.
I pray you get it figured out.
Excellent response! As a past Infection Control Nurse that sounds very plausible. I always when investigating my own aches and pains at the point of which there has been a change. That is how I discovered when I spent 8 months playing patty-cake with ENT, Neurology about severe vertigo/dizzy spells 2-3 times a day. After Pulmonary procedure for valves, pulmo changed my anti-coagulant from Warfarin to Eliquis, cause it is "easier" - no monitoring. Did deep dive research, found 5-6 case studies re: Warfarin vs Eliquis---2023 NIH research study revealed the patients once put back on Warfarin had symptoms alleviated. I stopped and Warfarin and haven't been dizzy since! Start at the time the changes occurred.
