After Mayo, how did you coordinate care with your provider back home?
Oh, I should've put my full question here.
I'm wondering about discharge from Mayo back to Ontario hopefully follow up recommendations for my healthcare providers inOntario.
Has anyone run into difficulties accessing follow up recommendations, treatments etc. upon returning home?
Any suggestions on how to present new recommendations to my physician?

Mayo providers work with you to develop a care plan that you can use with your local providers.
There is an older discussion on the subject. Even though it is talking more about the US, I would think it would be similar from Mayo to a Canadian provider.

Did you self-refer, or did your local provider refer you and send medical information?

2 ReactionsHopefully this is not just me but everybody. My local doctors , insurance and Mayo coordinated and took care of everything and still do till this day. I am copied and advised by both on everything but all three run with the ball seamlessly to this day. My local Dr. referred me to Mayo for my transplant and did the testing Mayo wanted done here in CO. And they sent all results to Mayo. Mayo approved me , insurance paid for all of it directly. I had access to see all the bills but rarely had to pay anything. If Mayo has recommendations they run them by my local doctors first. And then they jointly advise me. To this day local folks do my monthly bloodwork ,send results to Mayo and Mayo then advises me & local doctors same day of any changes. Mayo and my local folks are in contact with each other every month and I am advised when needed but I never have to drive on an issue or take the lead on anything. It's been that way 100% going on three years. Don't know if it is that way for everyone. If not for anyone please advise beekeeperj. Thanks
