After Evenity, must I stay on other meds?

Posted by callmekate @callmekate, Jan 8 10:51am

I am on shot #6 and my side effects have included itchy like mad for two days a week later, and having my spine sting down its entire length, as if the shower is hitting me too hard. At least, I blame Evenity for those. It seems no one really knows all it can cause.
When I finish my shots, I want to back away from all those meds that hang on to old bone, since it seems there is no way to ever stop those without suffering fractures. I have had 2 compression fractures in my -4.8 spine, which is why I agreed to the Evenity. I have also finally found a doctor, who works with a naturopath, willing to put me back on my BHRT at age 66, even with my heart issues. I have hopes that the combo of Evenity and BHRT will be enough to put me in a better place, going forward with weights and rucking, etc.
Does anyone have experience or suggestions for me?
Also, I have heard the first 9 months of Evenity build, the last three are for retaining old--and the new--bone. Is that true? Maybe I should stop after Month 9....

Many thanks, all!

Without additional antiresorptive therapy after evenity, bmd gains tend to be lost relatively quickly as bone resorption can rebound above baseline after 12mo treatment course. Estrogen in bHRT have antiresorptive effect and may help maintain bone mass, but whether bHRT alone after evenity is sufficient depends on your future fracture risk and medical history. Your endocrinologist can best assess what's needed based on your full risk profile.

Are you using estradiol patches or bHRT pellet?

I’ve had osteopenia/osteoporosis for 12 years now. I’ve been on bHRT for 14 years, starting in perimenopause at age 51.

I’ve also focused on strength training, impact exercise, and a healthy diet concentrating on adequate protein, leafy greens, veggies, fruit and complex carbs, and calcium, magnesium, and vitamin K supplementation.

I spent 5+ years on Fosamax which I now think was too long. It worked well for the first 2 years but stopped working after that. I made the mistake of relying on my PCP for osteoporosis care during those five years when I should have been seeing a specialist.

I was getting ready to come off of Fosamax after the recommended five years of treatment when I had a vertebral compression fracture. It was only then that I was referred to a specialist for treatment.

It’s been 2 years since my fracture and I have taken Tymlos, Forteo, and now Evenity. Everything else remained the same lifestyle-wise. The good news is that I haven’t fractured again.

I do have some side effects from the Evenity but will continue on it for another four months. I will do Reclast after to slow down bone breakdown for a couple more years. After that, I will stop and see how it goes. I want a couple of years off everything to see if I can continue to avoid fracturing.

My reasoning is that I did everything right but still had a fracture. I am always willing to try what the doctors suggest based on science-based evidence before I go against their recommendations.

This is how I feel and I know others have much different opinions about this. I also think that this is a serious and complex disease and the medications to treat it are no different than meds you’d take for other serious diseases like heart disease, kidney disease, high blood pressure, cancer, etc., in that all have side effects. There is a tendency to downplay the seriousness of osteoporosis as it doesn’t affect your life until you fracture. I was 100% the same until my fracture in ignoring it and not questioning anything.

I wish you the best in your decision as I know it’s a difficult one.

I've also had 6 Evenity shots. The technician at the infusion center told me that Evenity builds bone for the first 6 months & then retains the gains for the remaining 6--acting like a bisphosphonate. My insurance changed as of Jan 1, so I am waiting for approval to continue the course of treatment. Unless denied, I plan to finish the treatment and then give my body a break from any strong drugs. My HRT is also being modified to address bone health.

After a short break I plan to take Strontium Citrate. The success stories on Inspire.com have convinced me that it's worth trying. Then I'll get a REMS scan to evaluate next steps.

@mayblin I am using a .025 patch now. In two weeks, we will be assessing my estradiol numbers and I anticipate increasing that dosage. I'll post the results. Thanks for your input. I appreciate it.

@oopsiedaisy, I'll confess I didn't believe there was anything wrong with me when they told me I had osteo. I, too, had no idea how horrible this would be. We are done a grave disservice by not being fully informed of all its effects. I only ever heard about losing height and breaking a hip in old age. Older and wiser, I am asking better questions as I struggle with the future. Is it safe to stop Reclast after a year or two? Would that be a safe transition off of the meds?

@singingbones, I'll check out Inspire.com. I don't know much about strontium, but am ready to give any safe option a try. My REMS scan last year showed much better numbers than my DEXA, but I still fractured, which was disappointing, as you can imagine. It's so hard to know what to do.

@callmekate REMS remains controversial in terms of accuracy (but then so does DXA).

Thank you everyone for your post.
I mentioned in my first post, I suffered. With the doctors calling extreme osteoporosis. I had 5 bulging disks And my body crushed my rib cage , suffering for the rib fractures. I am a 62 year old male,.i was offered the Evenity injections for 8 months . I only did 6. I was given A prescription of a testosterone gel.
I tried hot yoga to increase flexibility. But I started getting more pain in my back. I have a sharp pain just below my ribcage on the left side that hurts most when lying down. I have had this pain since the contortions of my torsal. I was told that it was my nerves that caused the fractures.
I posted a reply to someone who said they were diagnosed as they're Osteoporosis was caused by epilepsy drugs.
I was taking Topiramate for over 10 years.
I am in Nicaragua and found a spine specialist at the spine trama center.
In Canada my GP said it will take a year to heal. The collapse disk started in April and I was feeling less pain by July of last year. I still has the sharp pain on my left side.
After reading, and not believing my doctor to wait a year. I found this health condition should be delt with asap. My ribcage now touches my hips. I forgot the name foe this condition, side-effects can be a pinching of the hip bone. Causing pain. I can only feel the bulging disks. I am not aware of how many are bulging. I watched a couple of videos on exercises, but they are painful,so I will try to keep moving with walking.
It is crazy that a person like myself suffered 5 compressed disks was very close to becoming a paraplegic. And my doctor said take the pain medication and relax at home.
I los 4" in height and have trouble breathing in damp weather.
I hope to find this thread to post the specialist discussion late next week.

@gpt, I'm down 3 and a half inches and have about half an inch between ribcage and hipbone. I'm sorry you are having so much pain. This disease is a misery.
