After Evenity

I will have my last injection of Evenity in two weeks. I don't want to have a Reclast infusion because I have had up to 5 in the past. I am concerned about starting Prolia for the remainder of my life. I am 70. I am considering Forteo because my insurance will pay for that rather than Tymlos. Any suggestions? I can't take bisphosphates. They wreck havoc on my stomach.

There isn't much information on Forteo after Eventiy. Some are saying it shouldn't be done in that sequence, but there aren't studies. In theory, it's the best sequence because (if as they say Evenity) is more antiresorptive than anabolic in the last months, then this timing of an anabolic would be effective than another antiresorptive. Trabecular bone integrity results with Evenity provide opportunity for Forteo to strengthen the bones.
Is your endocrinologist expressing a preference?

He is planning on putting me on Prolia. However, I am pretty sure I want more bone growth before Prolia. My spine was -3.8 when I started Evenity. I will get a Dexa in 2 weeks and my last Evenity shot is that same week. I will need a plan to begin a new drug in March. I am afraid to have Reclast. I already had 5 years of it. Thank you for your comment. It gives me some hope that I can still build more bone. My insurance doesn't cover Tymlos. I already checked out that option. I am pretty sure I have too much income to qualify for Tymlos help. I am a retired teacher. I saved too much money in my 401 K. (It doesn't pay to save. Haha) I need to check out how the $2,000.00 max out of pocket for Medicare Part D works. Thank you @gently for your hopeful response!

Pami , Prolia will help with increasing bone density. After one year of Evenity I too needed more bone. I have had one injection so far of Prolia and shortly after my second injection with have a DEXA to see if I will move onto Reclast to lock my gains in.

I have 4 more Evenity treatments. My question is, after finishing up all 12 treatments, at what point would you go on to something else? Would it be immediately, or would there be a time interval before restarting on a new medication?

First question: what does your endocrinologist or other bone expert recommend?
So many of us with very different drug sequencing.
You'll get great information about sequencing experience. In the meantime do plenty of research. It will help you develop a list of questions for your provider, such as
What is your rational for this drug recommendation and what would follow. What can I expect in terms of possible outcomes and possible side effects based on my medical history? etc

To me, an important question would be what is your T score as you complete Evenity. Your next text step might be different if you are still have osteoporosis than if you are now in the osteopenia range.

@susanew As @gravity3 asked I am wondering what your doctor recommends?

After I completed 12 months of Evenity my endocrinologist ordered a DEXA scan. I made significant gains in bone density in both my hip and lower spine. He explained that the gains with Evenity "fall off" if not "locked in" with another medication so within 6 weeks of completing the Evenity I had a Reclast infusion. My endocrinologist said the time frame between finishing Evenity and getting the Reclast infusion should be brief - no more than 6-8 weeks. The plan is for me to have another DEXA scan one year after the Reclast infusion (that will be in December, 2025) and then re-assess.

By the way, I had no side effects with Evenity or with Reclast.

I had a very poor Dexa prior to Evenity. I had a rheumatologist who dropped my insurance … so now my PPO is handling the Evenity referrals. When my Evenity comes closer to finishing, I will need a referral to an endocrinologist? for my continuing osteoporosis journey.
