Additional Treatment while on prednisone.
I was diagnosed in March for PMR at the age of 64. At the time I could not raise my shoulders and my hips, both inner and outer, were stiff also. I made an appointment with a young rheumatologist.(Graduated 1 year prior).
Two months prior to that i had a terrible lower back pull that still hinders me to this day, as i still can't tie my shoes. Additionally, I also tore my biceps muscle right before that. Ay my initial appt. the Doc told me the bicep tear and lower back and inner hip pain had nothing to do with the PMR. He told me the shoulder pain along with the hip pain (only outside girdle) was surely PMR. My blood tests were not elevated. He prescribed 10 mg of prednisone and wrote a PT script for my lower back and inner hip pain. After tapering to 7mg, my shoulders began aching again an he raised me up tp 9mg and a longer taper. This took care of my shoulder pain and i could agin raise them above my head. If I cannot taper again his plan is to put me on Kevzara.

After seeing a physical therapist I realized not a lot of them have treated PMR patients. After her initial evaluation she told me I was one of the most stiffest people she had ever treated. So, for a month now I have been seeing her two times a week and stretching faithfully two times a day. Both of us are think that I am not progressing like I should.
She now has tried dry needling on this lower back and i could only tolerate three needles the first session and one the second session. We are going to continue this if I can tolerate it. She also talked to several different therapists about dry needling a PMR patient. Again, there is not much information on this. Has anyone been thru this type of scenario or how about the dry needling?

I'm personally sceptical that dry needling would be good for PMR, but that's just my opinion. I had a course of dry needling several years ago because I was having frequent calf strains. In each session the physical therapist would insert the needle at least 20 times into each calf. Those sessions were agonizing. It was hard to walk afterwards and to drive home. As soon as I got home I would soak my legs in a bath with epsom salts. But the dry needling did help loosen up my legs and get rid of the adhesions in my calf muscles.
The reason I'm sceptical is that dry needling works on muscles, and from what I understand, PMR affects the lining of joints. I had several weeks of physical therapy while I had untreated PMR, and the PT only helped a little. Once I started on prednisone, the problems I was having from PMR and GCA cleared right up.
It sounds to me like your prednisone dose is too low. The Kevzara would be good, but it could take some time to take full effect. Google says up to 12 weeks. Good luck!
