81 year old who had prostate cancer 13 years ago psa risen to 36

Was treated for prostate cancer with radiation and hormones 13 years ago now psa has risen to 36 I have no symptoms and do not really want to go back on hormone treatment At my age of 81 should I consider treatment ?

I’m curious what you’re urologist advises. Perhaps a non-invasive MRI or a PSA PET scan? What was the velocity/doubling time of your PSA rise? I had proton therapy at age 68. Best wishes…

New to the group. I have had prostate cancer for ten years that metastasized to limp nodes and bone
For at least 3 years I have not had to use any meds. About a month during a scan they noticed my PSA started to raise a legion was found in the liver. A biopsy was done. My doctor ordered a PSA scan in the meantime he is his hitting it hard with Xtandi. I never thought the cancer would could back and the liver. I just turned 75 not something I ever thought I would fight again. Anyone gone through this or know of someone

Bone mets only Xtandi kept PSA down for 12 moths then oncologist put me on 6 Xofigio or Radium 223 injections PSA still rising at 31.4 Pray it does not go to soft tissue or organs Serious in liver Find a solution fast

Get on Zolodex as well to stop testosterones the fertilizer of PC

A starting point for discussions with your medical team could be the NCCN guidelines...The NCCN Guidelines for Prostate Cancer provide a framework on which to base decisions regarding the workup of patients with prostate cancer, risk stratification and management of localized disease, post-treatment monitoring, and treatment of recurrence and advanced disease. My question, did it suddenly rise to 36 or have you had other PSA tests. Standard of care says you would be having annual PSA tests so was this during your annual PSA test?

I have no symptoms and do not really want to go back on hormone treatment.

You do not have symptoms, yet, those may be in your future if you do nothing, question is how long before that and will something else happen in the meantime - a co-morbidity such as CV disease, diabetes, kidney, a car accident..all unknown.

At that PSA, standard CT and MRIs likely to show where you PCa is, that would be an important piece of clinical data as well as your clinical history, pathology report from your surgery which which give Gleason Score and Grade group

At my age of 81 should I consider treatment ?

In part, depends on your life expectancy since it could be up to eight years before the PCa begins to give you problems.

Probably, question is what, for how long...The imaging is a critical piece of that decision. If imaging shows say recurrence in the prostate bed, lymph nodes but no bones or organs, you could consider short term ADT plus potentially an ARI, for 6-12 months, then come off it PSA drops to and stays undetectable. If it shows recurrence in 1-3 locations, say PLNs or nones, SBT could be an option to knock it down, buying time.

So, consider imaging, informed by that information, decide on whether to treat or the "minimal treatment" needed for a defined period, then stop and actively monitor.
