60 Yr Old - % free PSA 10%, free PSA 0.26, Total PSA 3.98 in March

60 Yr Old -
Concerning: free PSA 0.26 and % free PSA 10% -- this seems bad according to normal ranges.

Total PSA 3.98 in March 2026, retest in April Total PSA 2.6

2025 - PSA 2.06

My PCP does not seem concerned. Should I go to a Urologist, and what next steps/tests would be most beneficial?

Hi,
Normally medical professionals don’t get too concerned until you get in the 4+ area. If your PSA history over several readings has been going up over the years it could mean something is going on but sounds like your good for now.

Thank You, and I really hope so!! My urologist seems to think nothing will show up.
The 10% free PSA is the concern, and a rise in the PSA over .75 several times. I've read about many people having the same situation as mine and not receiving good results. I will update after my mpMRI on Monday.

Update: I went to the urologist in Oak Ridge, TN.
He was reluctant, but he ordered an ExoDx and an mpMRI. My MRI is next Monday.
Is there anything I should ask or be aware of beforehand? Or after?

@oinc2526 and as Jeff mentioned below knowing the size of your prostate can help you calculate prostate density to put your PSA numbers in a context along with identifying characteristics of BpH or Prostatitis or suspicious lesions which can guide future decisions.
