Stage3b Prostrate cancer
In November of last year I was diagnosed with stage3b prostrate cancer. At that time I was told that the cancer was contained and that it had not spread. Now, six months later I’m told that I now have stage 4 prostrate cancer. Shouldn’t radiation and/or hormone treatments have been advised at that time?

2 years ago i was diagnosed with stage 1 prostate cancer by my urologist. He wanted to operate immediately because that is what he does, surgery. I got a second opinion at Moffitt Cancer Center. I had Stage 3b with a Gleason score of 7 (4+3). Glad I got second opinion at a cancer center.
How are you doing? What treatment did you receive for it. I also have your grade and score. Thank you.
Hi Darlynn, welcome to Mayo Clinic Connect. Are you in the process of making treatment decisions or have you decided what treatment to have?
Good morning! I read your comment, did not noticed it is years ago. Still. I hope you are doing OK. My husband has Stage 3a and since I am a medical professional , not practicing, though, and his caretaker, I really studied a lot, since my husband' s case was I would say tragic because of the late determination of cancer. When read about you, my blood was boiling :).
Let me ask you a question. Have you seen/read your biopsies results in full? Usually those are page or 2 long and for " normal" person not understandable. If you do not mind, I would love to see all that description, based on that the surgeon could determine is it really all over or the smart thing would be to send the patient to radiologist who will determine the next steps. If you like , please send to my email address. I wish you the best.
May 2021 my PSA was over 5. My urologist did a digital exam and said he felt something hard on the left side of my prostate. I had a biopsy done where 12 cores were removed and they came back negative. September 2022 my PSA was over 7 so I had another biopsy done by the same urologist. Guess what, I came back with stage 4 prostate cancer. I asked the urologist how that could be and he said in the first biopsy in May 2021 he only tested the left side where he felt something hard during the digital exam. Who the hell doesn't test the whole prostate? I asked my cancer doctor and he said the whole prostate should have been biopsied in May 2021. I had 9 weeks of 5 days a week of radiation treatments and my PSA is 0.32 and I have been on Lupron shots quarterly for about a year.
Do you still have a prostrate?
I am in Thailand living now for 10 years. Doctors are good here but I have to say when my PSA went from ,4, 3.1, 4.1, 4.3 he just said watch it with the annual physical. Well, this last one came back 6,9 and had the biopsy, came back all cores cancer, my Gleason was 8, had the surgery, awful chit, but after he took out many lymph nodes and the prostate, Gleason downgraded to a 7 and that was 3 months ago so first blood work is soon and praying for a 0, if above this
have to have a PMSA Pet Scan and if any spots found where prostate was, I am on Lupron for a year or longer, if a spot is found outside where the prostate was, I will do Targeted Beam Radiation for sometime as it poisons it and it dies and have to do fast as the spot would have been pin pointed. I feel the Internist that does all my physicals was very BAD not to move me to a Biopsy when my PSA moved above 4. Kind of pisses me off. I had only 1 3 month Lupron and it messed me up.
I was diagnosed with Stage 3B-N1-M1A and was immediately put on Zoladex, Abiraterone and three montns after that had 20 sessions of radiation. But in my case cancer had locally spread to lymph nodes and seminal vesicals.
Please expand on why 13 month Lupron messed you up.
I only took 1 3mo Lupron shot and had the prostate removed not that long after the shot. Now,
I am just waiting for the blood work, my first since the surgery on Aug. 3rd. That first BW will give us the PSA reading and if 0, just watch then with more BWs every 3 months. If the PSA reading is up any, an immediate PMSA Pet Scan and if any spots are found, if the spot is inside where the prostate was, Lupron shots for a year to year in a half. If a spot is outside where the prostate was, then Targeted Beam Radiation to poison any cancer spot. I sure hope it is 0.
