It sounds like she has been through a lot of testing. Sometimes when you are at "the end of your rope" with unknown issues, it is time to go to a center where a multi-disciplinary team works together to find answers. Have you considered contacting Mayo Clinic (http://mayocl.in/1mtmR63) or a large teatching university hospital near you?

So very sad to hear of the situation your wife and you are suffering .
My depression was diagnosed as Anxiety due to the Pandemic which was possible. I pushed further and was diagnosed with overactive thyroid which is now under control due to fitting medication.
I am hoping for better days for you and your wife.
