Questions about MRI report

Posted by scottrb43 @scottrb43, May 9, 2023

I had an abnormal PSA for my age and so I was referred to an urolgist who did a digital rectal exam and didn't feel anything so I had an MRI done and it says there is a subtle T2 hypointense lesion of the left posterior prostate gland measuring 11mm it is associated with slightly increased DWI signal and decreased ADC signal. Enlargement of the prostate gland measuring 5 by 4.5 by 3.6 cm They can't get me in for a biposy fo several weeks. SHould I be worried that it's cancer? I not only have incontinence and impotence issues but I have had a dull aching pain in my upper thighs and what feels like a blood clot in my calf. Any advice would be greatly appreciated, Thanks!

Your symptoms could indicate that you have serious health issues and only a biopsy at a top rated cancer facility can give you a competent answer. Everyone here has been where you are now and I am sure that everyone wishes you good health and good luck!

Biopsy will tell if its cancer or not and what gleason grade it is. Pain in thigh and calf your Primary Care Physician should address. I wouldn't think prostate issues would cause that, but I'm not in the medical field.

Sounds like you are doing the right thing, except the worrying. I was in a similar spot, and was very worried until I learned that if it is cancer, that I caught it before it spread to other parts of my body and usually progresses very slowly. Having a DRE with good results. Is great, mine was bad news. I've been on hormone therapy for 8 months now, Testosterone and PSA are undetectable , side effects are minimal.

If the biopsy does bring the dreaded news, hopefully your Doctor will order the PSMA CT/PET scan that is a new type of imaging test that can help doctors learn whether prostate cancer has spread. This imaging scan helped alleviate a lot of my fears after the biopsy confirmed the cancer.

If you do have a biopsy that shows cancer, you might want to get a decipher genetic test using your biopsy material to see how aggressive the cancer is. Doctors use them, and it may give you, as it did me, some level of comfort.

I'm not a doctor, but my research on T1 and T2, on an MRI scan. T1 means that there is water there that shouldn't be there, T2 indicates water and fat that shouldn't be there. It could be an indication of cancer, or due to injury or infection. According to Bayes theory of probability, if you only test for one outcome and there are more possible outcomes. The results are flawed. Interesting yes. I hope this helps.

Based on my experience, I would ask your doctor about a PSMA PET scan. Such a scan is reportedly much more revealing than other scan types. My PSMA PET scan revealed endocarsinoma in a gland very close to my heart. Very small. Led to chemo with ADT and Nubeqa.

@scottrb43, have you had the biopsy? Any update?

The incontinence and elevated PSA may be due to BPH. You can try anti inflammatory drugs to see if the problem is reduced. But at the end of the day, you can have enlarged prostate and cancer. Biopsy may not identify the cancer. PSMA PET scan is more certain. That plus MRI is 98%.
