Extreme outer ear pain: What can it be?

I am 73 years old and been dealing with this most of my life. For the past 4 or 5 years it has gotten unbearable. I get very little uninterrupted sleep. No doctor can explain this. I will read comments for any advice.

I take Meloxicam for inflammation and am now sleeping through the night every night.
