Hi @alh123, the Pain Rehabilitation Center in Arizona is on the Phoenix campus. Check-in is in the Mayo Clinic Phoenix Building.
The contact phone number is 480-342-6240.

Did you receive the information packet? If you are registered as an attendee, you should've received communication from the Pain Rehab Center by phone, the patient portal and mail. I want to ensure that something was not missed in the process. Please let me know.
