@cclaus Clearly something medical is going on with your husband . I understand why you are scared and want to understand what "white matter disease" means. This isn't really a diagnosis, per se, but rather a description of changes that are happening in the brain.
Does he have another condition or diagnosis that he is taking medication for? Has your husband been diagnosed with multiple sclerosis (MS)? You mention legs, balance issues and hearing loss. Does he have weakness in his legs? Or numbness? These could be a problem with the nervous system (central nervous system or peripheral nervous system).
White matter disease refers to what appears as as white or light in the brain. As @ldrlaw explained, white matter is the myelin sheath that covers axons (nerves) that send signals from brain cells (neurons) to other brain cells and myelin helps to conduct nerve impulses. Here is a good description from Cleveland Clinic.

Thank you for the responses. The Cleveland Clinic article was a good one. He does have other conditions but are well controlled and have been with medications. High Blood pressure and cholesterol medications. His blood pressure is consistently low, like 110/68. His medication for cholesterol is a minimal dose. He is borderline with the sugar/diabetes. He does have problems with circulation in the legs from the knee down. I put a pressure pump on his legs daily. I think they said his lymph nodes don't work well in his legs. He has been scanned a few times for blood clots and nothing is revealed there.
I guess our next step is neurology or cardiovascular. I do appreciate the discussions.
Hello @cclaus and welcome to Mayo Clinic Connect. Hearing this diagnosis has to be concerning and, no doubt, comes with a lot of questions. I am glad you have reached out for support here.
You will notice that I have moved your post into an existing discussion on the same condition so that you can more easily connect with others in the community. You can now find your post here:

I see members such as @ldrlaw and @naturegirl5 have joined you already. I'd also like to call in members @narus and @awilst who have experience with this condition.
Are you scheduled for a follow up with Neurology and/or Cardiovascular like you had mentioned?
My husband was diagnosed with White Brain Disease over a year ago at the Cleo Roberts Center outside of Phoenix, AZ. I had become concerned way back in 2012 and made the appointment for him there. IF this is the same thing you're dealing with, my understanding is that it has to do with deterioration of the myelin sheath in the brain. Per Cleveland Clinic, the myelin sheath is a protective membrane that wraps around part of certain nerve cells. Myelin affects how fast signals travel through those nerve cells. It's called White Brain Disease as it shows up white on scans. It can go on for decades and in my husband's case affects executive functions but if you met him you wouldn't realize he had a problem. I had worked with seniors my entire life and owned a group home - never heard this term before he was diagnosed.
