Why isn't hormone therapy used in all prostate cancers?

An observation and insightful comment by one group member here: "Lupron/Eligard both shrink the prostate and stops the production of testosterone which feeds the growth of prostate cancer." If so, why don't all treatment options of prostate cancer include hormone therapy? I am waiting on a second opinion for my treatment (Lupron + EBRT) from Mayo hoping I can do without the often dreaded side effects of hormone therapy. I realize one size doesn't fit all, but why is hormone therapy used in some prostate cancers and not in others?

From what I am reading about Lupron there are serious side effects from Lupron. That's no small consequence. I haven't seen anybody talk about the interval between the shots, and when i think about it maybe that interval can be increased so as to diminish the horrible side effects. Now that i can see the overall picture of the disastrous effects of Lupron the what about the other anti androgen drugs? There re quite a few that are mentioned, but no one has had them. So again, it seems to me that there needs to be greater consideration about the side effects of Lupron before going too quickly into its use. The issue now is that there are more anti androgen drugs, that might be able to be combined with a lower dose of lupron that might diminish the side effects. I would love a comment on this.

I just want to add to what i have already said about combining drugs so as to reduce the horrible side effects. What comes to mind might include progesterone as another androgen suppressant. Where is the downside, if any?

On Orgovyx for prostate cancer, Gleason 4+3, radiation therapy completed 3 months ago. 4 months of ADT treatment is planned. I experienced complete heart block 3 week after starting requiring a pacemaker but I had some prior heart symptoms so it may have been a coincidence. Otherwise just the usual sexual side effects and hot flashes from ADT. I was started on citalopram for underlying depression and that has had a clear benefit in lessening the hot flashes. I concur with those recommending exercise and a healthy diet and have been doing both of those. So far I would probably have chosen this course of action again but I hope the ADT side effects will abate soon after stopping.

I had EBRT (45 sessions) in late 2015. PSA was almost 11, and Gleason 3+3=6.
The Radiation Oncologist discussed hormone therapy, and advised against it as studies were showing it had virtually no difference in outcome.
Last check PSA (last fall) was 0.19.

Recently experienced complete urine retention, ER visits, times 2, Foley, etc. On Silodosin for several weeks now, and self Catherine 4 to 6 times a day. She originally had me take tamsulosin, which I can't tolerate.
I have no problem with normal urination currently, but the Urologist wants to eliminate any urine retention. Will know more after flow test, PVR, and cystoscopy in a couple of weeks.

I would like to read your doctors authorities that hormone treatment has no or little effect on survival It is a very common treatment for most PC and it is often given with other treatments even chemo. If I can read some authorities on this I will discuss with my UCLA doctors.

You can go to the National Institute of Health where they have a report on the history of ADT and research on when it's effective for treatment of prostrate cancer. The authors are Zaid Siddiqui and Daniel Krauss. Also, the Prostrate Cancer Foundation has information on it. They also have a section on Precision Treatments for prostrate cancer that looks at your DNA to see if you have gene defects causing the cancer and possible new treatments that can be used that don't include ADT nor radiation or surgery.

I got the 6 month hormone shot 14 months ago for prostate cancer and still suffering the side effects. I feel I was not told how bad this treatment is. I have no energy and no sex drive and it has ruined my life. My doctors will not give me any answers on why or when my testosterone will return. Don't understand why a 6 month last 14 months. I wanted to only do the 3 month shot and was told not to and take the 6 month. Has anyone else had any issues like this from the shot if so please let me know when you started to feel normal.

I have no energy and no sex drive and it has ruined my life. My doctors will not give me any answers on why or when my testosterone will return. Don't understand why a 6 month last 14 months. I wanted to only do the 3 month shot and was told not to and take the 6 month. Has anyone else had any issues like this from the shot if so please let me know when you started to feel normal.

I couldn't agree more with your statement. I received my one and only dose of Lupron in June of 2022 and was not given all the information in regard to how life changing this would be. Although the Radiation/ oncologist strongly encouraged me to continue with the Lupron injections I opted to stop after the first dose. The combination of hot flashes, mood swings and overall depression turned me into someone I didn't even recognize. Never again would I agree to that course of treatment. The radiation (31) sessions was difficult but it has made a significant difference so far. Not a time in my life I would choose to repeat if given a choice. My psa 3months after radiation was 0.11 and now after another 3 months is at 0.03. I feel great and pray each day that I will never have to go through that again! Best of luck to all those that follow in these steps.

Hi George, they wanted me to do the 3 month and I told them nope I’ll do it monthly n after two months found out about orgavix the oral tab n allows test to return much quicker n thank God I did. After two months of lupron n the 5 months of orgavix it came back within two weeks, actually higher than it’s ever been at 585 but so did the psa to 6, so got back on the adt along with zytiga n psa went back down to.76. After a month I found out about ivermectin n did 6 doses of 48mg/ every other day n my psa dropped to its lowest ever at.11, proceeded to get off adt n zytiga, test returned after two weeks but the psa remained low below 1, @.90…, I’m taking my second dose of ivermectin this time 3 days concurrently @42 mg/ daily and will keep you posted..
