Metastatic Prostate Cancer: Where do I go from here?

PSMA PET scan revealed radiotraser uptake in the right 7th and 9th ribs consistent with osseous metastasis. . Where do I go from here. I am 82.

I try to keep researching without it becoming overwhelming.

Had radiation 2018 then it metastases in 2021 so had L2 lumbar nuked and back on Zolodex 2 years later it increased but slow growing in L1 and hip so they want me on Enzalutamide Note it's the latest COPY Xtandi was granted an FDA fast track and later approved in metastatic castration-sensitive prostate cancer (mCSPC) by late 2019. In May, the oral treatment also scored a European nod for that indication, which carries a grim median survival of just three to four years, Astellas says. U.S. regulators already approved Xtandi in mCSPC and nonmetastatic CRPC (nmCRPC).
For its part, androgen receptor inhibitor Erleada, J&J's follow-up to aging Zytiga, was also approved in mCSPC in 2019 and in nmCRPC a year before that. (so Phizer Enzalutamide or Extandi seems superior outcomes

I’m 88 and diagnosed July 2022. Big surprise! We’ve been in PCLO clinical trial for 20+ years. On ADT therapy at Mayo, Leuprolide inj (3month) and oral abiraterone 250 mg daily. Feeling fine, few hot flashes, definite weakness overall. Good luck.

I was Gleason 7 4/3 and had rp 4/18. Bcr 2021. 36 salvage rt then one undetectable. Yesterday psa .26 after last undetectable 5 months ago. Any thoughts on what to know other than following up w my docs?

Itterac, curious if your post RP pathology showed cancer contained to prostate or whether cancer was found in Lymph Nodes. Also, was there any perineural invasion? I had Gleason 7 (4/3) and I am trying to assess my reoccurrence risk going forward. FYI - My post RP pathology did not find any cancer in the lymph nodes that were removed and no perineural invasion. I know this is only a point in time assessment, so want to continue assessing experiences of others like me.

My RP was on November 3, 2022. They took the prostate/seminal vesicles and 6 lymph nodes.

Oncologist has now put me on Xandia pills with Lupron. Suppose to get my first belly shot 9 January. Has anybody been on Xandia? Doc will do more blood work end of January. Lupron shot is for 6 months.Doc said I would be on Xandia and Lupron for ever and ever . Another Doc said intermittent pending psa level and how well that I am taking the meds. Any info on Xandia? Thanks.

I was on Xandia and I’m now on Ebearla. I tolerate Ebearla better.
I do have been told I will be on Lupron and one of these chemo pills forever. I see this as treatment and not cure. Unfortunately, all these doctors want their revenue streams to stay in place. The. Xandia gave me lethargy , a light, mental fog, mild nausea. I lived life normally. No one could tell I was undergoing treatments. I still played golf and rode the motorcycle and played pickle ball. Each body tolerates this differently, just pay attention to your day to day.

Thanks for the response it is appreciated . I have had mo reactions to Casodex. Issue is the Oncologist I started with in Richmond only prescribed 30 days of Casodex and midway start a 6 month injection of Lupron. I was moved to Tappahannock to shorten my drive for treatment. The new Oncologist wants me on both Xtandia and Lupron. Will see how the old bod tolerates the meds. I plan to ho bavk to the Y for more weight bearing exercises. Since other half has dementia most of my time is taking care of her. Thanks again for responding. Will let you know how things turnout. By the way used to ride a Honda until it was stolen.

Hi, keep up the exercises and if you can walk 30 minutes a day. I bless you for taking care of your spouse! There are and half dozen different medicines like casodex. So options there, lupron options are tougher for your body to tolerate, so try to stay with Lupron if you can. I do want to stress this is treatment and not cure. Stay the course and stay strong. I have run into many doctors who focus very little on me and quite a bit on what treatment regime I will be on. Often they don’t know my name or my chart. This is the sadness in Drs today.
