Looked this up and repetitive motion, like sitting alot, bike riding and/or other sports can aggravate it. Like most things, stretches can help relieve the referred pain from tight glutes, hamstrings (!) etc. Do get a referral with PT after your next shot. It's the best time to deal with this. Ice packs, bio freeze, lidocaine patches or other topical fixes can help. Maybe a donut or other type of pad would help? https://www.ncbi.nlm.nih.gov/books/NBK482285/

A Colorectal surgeon advised me that the pain, in my case, is caused by a muscle surrounding the rectum, vagina, lower urinary track, etc. in that area of the body. (I had my colon and rectum removed.) He said that when the muscle tightens it can cause pain. He recommends physical therapy and perhaps a botox shot, when needed, It is a difficult journey trying to find a diagnosis and treatement for the pain.
