Is facial twitching due to neuropathy?

Posted by Karen @fue, Sep 19, 2023

Tests show I have motor and sensory neuropathy, definitely in arms and legs, with nerve pain in front torso. For a few years I have had facial twitching, usually daily, moving between eyelids, under eye, lips, chin and the tip of my nose, only one area at a time. These are irritating but only noticeable by me. I sometimes have mild headaches lasting all day. I wake a lot in the night and my REM has recently become extremely low. Now I have developed a subconjunctival haemorrhage in my right eye. Are these all connected?

Diagnosed with polyneuropathy. Facial twitching getting stronger on both sides - lower lip, chin, under eyes and upper eyelids. Neurologist checked me for Bells Palsy. Awaiting MRI Head results. Any more ideas please?

Hi Karen @fue, There are a couple of discussions on hemifacial spasms that you might want to view. Here's some information on the condition.

"The condition may be caused by a facial nerve injury, or a tumor, or it may have no apparent cause. Rarely, doctors see individuals with spasm on both sides of the face. Most often hemifacial spasm is caused by a blood vessel pressing on the facial nerve at the place where it exits the brainstem."

Have you thought about seeking help at a major teaching hospital or health facility?

Hi @johnbishop
Thank you very much for your reply.
I have read about hemifacial spasms but this is both sides.
I would say it is mainly right sided but I do get twitches around the left eye too. On a daily basis it seems to move from left under eye to right under eye and chin and lip. Both under eyes are getting stronger. I don't drink alcohol. I drink 2 coffees a day max before 11.30 am. I don't sleep too well.
At the moment, due to the NHS waiting list initiative I am very lucky to have a private Neurologist from Medicare. I saw him in August and I thought he was extremely thorough. He checked for Bells Palsy. Ordered more electrical testing and an MRI head which I had under NHS on 26 Aug, this is still unreported, I checked today. And I used to work for that Radiologist!

Hoping you find some answers. I had just read the article and saw the sentence which leads me to believe hemifacial spasms can occur on both sides but it's normally on one side or the other.
"Rarely, doctors see individuals with spasm on both sides of the face."

I have hemifacial spasms for 4 years now. Just started small amounts of Botox injections every 4 months a year ago. My neurologist seems to think that I now have Botox complications of the Botox spreading! The left side of my face that has the spasms & Botox injections is getting more & more stiff to where my smile is crooked & it appears I talk out the right side of my mouth. The left side spasms are worse around the side of my mouth. Anyone else having this problem???

Have you had a skin biopsy to determine what type of neuropathy you have? That information would help you research the facial twitches, which are definitely related to some forms of neuropathy. My neuropathy is a “ stocking and glove” progression, starting at hands and feet. Others have more scattered forms.

Hi
No, no biopsy. I was told, after tests, that I have length dependent axonal neuropathy.
Yes, mine is stocking and glove. Definitely up to knees and elbows now. So far, only occasional numb patches on thigh. Confirmed motor and sensory. Occasional nerve pain in torso. Scattered petechiae increasing on torso.
Facial twitches, subconjunctival haemorrhage in right eye, more mild headaches. Occasional migraine. Neuropathic itch.
I'm so worried this is going to become autonomic.
