Old pain being relived again

I am at home rather than at work because of an old injury, or at least I was told it was an old injury of a stress fracture in my lower right back vertebrae. It started when I was 14 years old. The last time my back went out I was 21 years old which was 33 years ago. I have not felt this pain since I was 21. I have been to other orthopedic doctors and they said they saw no area in the lower spine where there had been a stress fracture yet I saw the x-ray myself when I was 14. I do however have DISH disease, and I am now wondering if the pain I am experiencing is from that and maybe it was even back then despite the fact that they don’t have a lot of literature of young people having DISH. The pain goes from the lower back and over the hip into the front of the leg slightly. I know I have found literature that states the medial gluteal tendon has been affected by DISH and that’s what I’ve been being treated for at physical therapy for the last six weeks. I had to switch to aqua therapy due to land physical therapy causing more problems for me. Any words of wisdom? Has anyone tried EPAT?
I do look at the website often and hope that there is something I can help all of you with from time to time. I have replied to a few things but I don’t have experience with others. I wish I could be of assistance to you all.

Sorry to hear you are suffering such a diverse group of painful symptoms. I have also read your earlier posts, and it seems your main source of pain at this time is medial gluteal tendinopathy.
Tendinopathy can be miserable since it occurs in the joints we rely on for everyday activity.
Since you seem to not be getting relief from traditional PT, it may be time to explore EPAT. Here is what Mayo Clinic has to say about it.

But - I think you need a clear diagnosis of whether this is tendinopathy or DISH - that is, soft tissue inflammation rather than calcification of the tendon. I believe this could be determined with imaging, since calcification shows on x-rays.

Do you have a doc who can take a closer look at what is going on?

Thank you for your reply. I have had many MRIs this year and just had x-rays yesterday that were horrifically painful. Whatever is going on in my back right now is worse than the DISH that has been confirmed & that I am treated at pain management for. I believe aqua therapy was helping but land PT was to stressful on my body. I found an article that says that medial gluteal tendinopathy has been found in DISH patients. From what I have read about DISH, the ligaments and tendons become hardened along with soft tissue.
