Fabulous athenalee! Congratulations! Wow that’s great news. So far out here in Oregon and Washington, I am getting the run around. Decided to reach out to my Senator to see what she can facilitate. Please let us know how things progress.

If you scroll down on this website, under healthcare providers is a letter authorizing Evusheld under FDA guidance - https://www.oregon.gov/oha/covid19/Pages/Healthcare-Partners.aspx
There’s a link on the Washington Health Department providers website too, but the link is broken.

You might try reaching out to your state health department. I downloaded the form and info on the VT Health Dept Covid Website, under physician alerts. Filled out the form as much as I could for my PCP. Unfortunately, since I have Covid it’s on hold.
